About this file

This is a Department of the Interior Past Performance Questionnaire (PPQ) template for evaluating contractor performance on federal construction projects. The form is designed to be completed in two sections: Section A, which the offeror completes with project details including title, location, company name, CAGE code, key personnel, and contract dates and amounts; and Section B, which the evaluator (project owner or reference) completes with detailed performance ratings and commentary. Past performance questionnaires must be submitted to Ashley Warcewicz at ashley_warcewicz@ios.doi.gov by the deadline specified in the solicitation, with offerors permitted to submit between 2-5 questionnaires covering projects completed within five years of the evaluated contract's completion date.

The questionnaire uses a five-point rating scale (Outstanding, Above Average, Satisfactory, Marginal, or Unacceptable) to evaluate nine key performance areas including contractor-client relationship, corporate management and integrity, quality control, schedule performance, and overall satisfaction with final deliverables. The evaluator must also address whether cure notices, terminations, or other corrective actions were issued; whether they would award future contracts; and provide an overall performance rating with supporting comments. The information collected is designated as source selection sensitive and cannot be used for promotional purposes or to imply government endorsement. This template is directly associated with solicitation 140P5326R0011 for the rehabilitation of Fee Memorial Church exterior in Nicholasville, Kentucky, a 100% small business set-aside valued at $45 million with a firm-fixed-price construction contract and 180-day performance period.
