The form requires contractor employees to acknowledge and comply with strict confidentiality and information security protocols regarding controlled unclassified information (CUI), procurement-sensitive data, proprietary business information, and personally identifiable information (PII) that they may access during contract performance. Signatories must certify they will use such information solely for official government purposes, disclose it only to authorized personnel with a legitimate need-to-know, maintain safeguarding standards per 32 C.F.R. Part 2002 and applicable Department of Defense policy, return or destroy information upon contract completion or officer request, and immediately report any unauthorized disclosures. The second section requires certification of conflict of interest (COI) status and includes the Contractor Conflict of Interest Disclosure Form (ADAP Form 209-506-1). Contractors must certify they have no direct or indirect financial interest in their contract work and must disclose any conflicts involving themselves, spouses, minor children, or business affiliates. The form specifically prohibits participation in bid and proposal reviews, technical evaluation panels, specification drafting, and budget work related to competing contractors or affiliated entities, and requires disclosure of any changes to financial interests within one business day of discovery. The form is a template requiring completion with specific information about the solicitation number, program, organization, work description, conflict assessment, and mitigation plans, with signature authority required from a responsible company official.
