About this file
The documents are a set of forms and a process flowchart for Transient Contractor Hazardous Materials (HAZMAT) Tracking for the 45th Space Wing at Patrick Space Force Base/Cape Canaveral Space Force Station. The primary documents include a HAZMAT Usage Worksheet and a Construction Contractor Spill Notification Form, which contractors must complete to track and report hazardous material usage and potential spills.
The HAZMAT tracking process involves multiple steps from contract award through project completion, including contractor submission of proposed hazardous materials lists to the Contracting Officer, review by the Installation Hazardous Materials Program (IHMP) Manager, authorization processes, quarterly reporting, and final closeout. The process requires contractors to prepare Chemical/Hazardous Material Request/Authorization Forms, attach Safety Data Sheets (SDS), and input data into the Enterprise Environmental Safety and Occupational Health Management Information System (EESOH-MIS). This systematic approach ensures comprehensive tracking, reporting, and management of hazardous materials throughout the contract lifecycle.
