About this file

This is an evaluation scoresheet for the Utah Communications Authority ("UCA") Geyser Peak New Site Build Project RFP, a state procurement for construction and equipment installation services at a remote communications site in Sevier County, Utah (GPS coordinates 38.511960, -111.460180). The project encompasses a multi-discipline off-grid communications tower and shelter installation requiring site preparation, tower foundation and erection (Sabre S3TL with 45-foot northeast leg ice shield and four ground bus bars), modular shelter installation (~12,000 pounds), propane tank and fuel line installation, solar photovoltaic systems (three sectors with twelve panels each), microwave and radio antenna and transmission line systems, ice bridge installation, facility grounding, and comprehensive testing and closeout documentation. Questions are due by May 19, 2026, at noon MT, with proposals due by May 22, 2026, at 10:00 AM MT via the U3P portal. Materials for the project will be picked up from the UCA Warehouse and UDOT Road Shed near Fish Lake (GPS coordinates 38.473200, -111.832516). The substantial completion date is July 31, 2026, requiring contractors to coordinate mobilization, foundation curing, equipment delivery and installation, testing, demobilization, and closeout within the specified timeframe while accounting for USFS coordination, weather conditions, road access limitations, and UCA material procurement dependencies.

The evaluation scoresheet uses a 1,000-point total scoring framework with cost integrated into the technical evaluation pursuant to a written waiver executed under Utah Code 63G-6a-707(7)(b). The technical scoring allocation is 750 points across five criteria: Contractor Experience and Past Performance (200 points), Project Team and Subcontractor Qualifications (100 points), Technical Approach and Work Plan (200 points), Project Schedule achievement (150 points), and Safety and Quality Assurance Program (100 points). Cost constitutes 250 formulaic points calculated by the procurement officer using the statutory formula: Cost Points = 250 × (Lowest Grand Total ÷ Offeror's Grand Total), with cost line items derived from 16 scope-area lump-sum prices submitted in each proposal's Appendix C. Three independent evaluators score technical criteria and provide written justifications; the procurement officer calculates cost points without subjective scoring. The final ranking averages the three evaluators' technical scores, adds calculated cost points, and flags which responsive offerors meet the 800-point minimum threshold required by RFP Section 12, with the highest-ranked offeror meeting the threshold receiving the award recommendation.

UCA Geyser Peak New Site Build Project — Evaluation Score Sheet
Overview
This workbook captures committee evaluation of proposals submitted under the UCA Geyser Peak New Site Build Project RFP. The scoring framework is 1,000 points total, with cost integrated into the technical evaluation under the waiver authority of Utah Code § 63G-6a-707(7)(b). The Procurement Official has executed a written waiver, which is on file with the UCA Procurement Unit and is reproduced in Section 9 of the RFP.

Workflow
1. The procurement officer fills in the Offerors tab with the names of each responsive offeror.
2. Each evaluator opens their dedicated tab (Evaluator_1, Evaluator_2, Evaluator_3) and scores each offeror against the five technical criteria (Criteria 1–5, total 750 points possible per offeror). Evaluators provide written justification for each score.
3. Cost is NOT scored subjectively. The procurement officer enters each offeror's 16 line-item lump sums into the Cost_Calculation tab (one column per offeror). The workbook auto-sums the line items into a Grand Total and applies the statutory formula: Cost Points = 250 × (Lowest Grand Total ÷ Offeror's Grand Total). The tab also captures the offeror's stated Grand Total from the bottom row of Appendix C and flags any inconsistency with the computed sum for procurement-officer attention.
4. The Summary tab averages the technical scores across all three evaluators, adds the calculated cost points, totals out of 1,000, flags whether each offeror meets the 800-point threshold, and assigns a Rank.
5. Each evaluator must sign and date their tab. All tabs together comprise the evaluation record retained under UCA's records-retention obligations.

Statutory Authority
Utah Code § 63G-6a-707(7)(a) ordinarily requires that evaluation committee members not have access to cost information until after technical scoring is recommended. Subsection (7)(b) permits a procurement official to waive that prohibition by signing a written statement of best interest. The UCA Procurement Official has executed that waiver for this procurement; the waiver is on file with the UCA Procurement Unit and is set forth in Section 9 of the RFP. Accordingly, cost is integrated into the overall evaluation as the 250-point Cost component.

Cost_Calculation

Cost Calculation — Line-Itemized (Procurement Officer Only)
Per the § 63G-6a-707(7)(b) waiver, cost is integrated into the evaluation at 250 points. Cost is not scored subjectively. Enter each offeror's Lump Sum Price for each of the 16 scope-area line items from Appendix C of their proposal (one column per offeror). The workbook auto-sums the line items into the COMPUTED GRAND TOTAL row. Also enter each offeror's STATED GRAND TOTAL (the figure they wrote on the bottom row of Appendix C); the consistency check row will flag any mismatch for your attention per RFP Section 9. The COST POINTS row applies the formula: Cost Points = 250 × (Lowest Computed Grand Total ÷ Offeror's Computed Grand Total). If you elect to score an offeror at zero cost points due to a material inconsistency (RFP Section 9), overwrite their COST POINTS cell with 0 and add a note.

Summary

Evaluation Summary and Final Ranking — Geyser Peak
Final scores: average of the three evaluators' technical subtotals (out of 750), plus the calculated cost score (out of 250), for a total out of 1,000. The 'Meets Threshold?' column flags whether the offeror cleared the 800-point minimum required by RFP Section 9. The Rank column auto-assigns rank by Total score (1 = highest). The recommended award per RFP Section 12 is the responsive offeror with Rank 1, subject to meeting the 800-point threshold.
