About this file
This is a Location Certification form issued by Broward County, Florida for use in Request for Proposals, Request for Qualifications, or Request for Letters of Interest solicitations. The form enables vendors to certify their eligibility for local preference and location-based tiebreaker considerations under Broward County Code of Ordinances Section 1-74. Vendors must submit the fully completed certification form with required supporting documentation by the solicitation end date or within three business days of the County's written request to qualify for local preference points or the location tiebreaker. The form requires vendors to select one of five options: Local Business status; Local Business and Locally Based Business status; Local Business and Locally Based Subsidiary status; Joint Venture composition; or non-local business status. For Options 1-4, vendors must demonstrate continuous maintenance of a physical business address within Broward County limits for at least one year preceding the bid posting date, proper zoning compliance, ownership or legal right to use the property, and day-to-day operation from that location. Locally Based Business and Subsidiary designations require additional certifications regarding equity ownership percentages, employee primary business address locations, and management coordination of activities from the local location.
The form applies to the Risk Management and Claims Administration System solicitation (Reference BLD2131488P1) with a proposal due date of November 13, 2026 at 2:00 PM EST. The contract structure includes a five-year initial term with five one-year renewal options. The County may request additional supporting documentation including proof of property ownership or use rights, corporate organizational charts identifying parent entities, executed joint venture agreements, and other documentation regarding the location from which vendor activities are directed and controlled. Vendors must certify their intent to remain at the listed local business location for the contract duration, including renewals and extensions. False, misleading, or inaccurate information submitted may result in vendor suspension or debarment and potential termination of any awarded contract. The OESBD procurement preferences for small business enterprises and county business enterprises apply where qualified vendors respond, though no specific set-aside percentages or contract value estimates are provided in the pre-solicitation notice.
