About this file

This is a Vendor Questionnaire and Standard Certifications form issued by Broward County, Florida's Purchasing Division for use with Requests for Proposals, Requests for Qualifications, or Requests for Letters of Interest. The form requires vendors to provide comprehensive business information including legal business name, Federal Employer Identification Number, Dun and Bradstreet Number, principal place of business, authorized contacts, and details regarding principals, owners, officers, and majority shareholders. Vendors must disclose any current affiliations with other organizations, history of debarment or suspension, failures to complete services or deliver products within the past three years, bankruptcy filings, surety interventions or bond claims, and contract terminations. The form also requires vendors to certify their participation or non-participation in the solicitation development process. Proposal responses incorporating this questionnaire are due November 13, 2026 at 2:00 PM EST for the Risk Management and Claims Administration System contract, with an optional pre-proposal conference scheduled for October 23, 2026. The contract will have a five-year initial term with five one-year renewal options.

The questionnaire mandates standard certifications including drug-free workplace certification under Florida Statute 287.087, non-collusion certification requiring disclosure of any Broward County officer or employee relationships, public entity crimes certification confirming vendors are not on the convicted vendor list, and scrutinized companies list certification verifying vendors are not engaged in prohibited business activities related to Sudan, Iran, Israel boycotts, Cuba, or Syria. The form specifies no pricing information or estimated contract value in the pre-solicitation notice. Vendors must register in BPRO, the County's electronic bidding system, to receive notifications and submit proposals electronically. Questions regarding the solicitation are due October 27, 2026 at 5:00 PM EDT. No bonding requirements or established goal participation set-asides are specified, though Office of Economic and Small Business Development procurement preferences for small business enterprises and county business enterprises apply where qualified vendors respond.
