About this file

This document contains the Evaluation Criteria for a Group Long Term Disability Insurance contract for Broward County, Florida. The document outlines a comprehensive 100-point scoring system for vendors proposing to provide long-term disability insurance services to approximately 6,115 benefit-eligible county employees. The evaluation criteria cover seven key areas: vendor experience and professional personnel (23 points), quality of service and customer service (13 points), implementation (5 points), performance measures (10 points), plan design (24 points), location (5 points), and pricing (20 points).

The pricing component will be evaluated using a formula that calculates points based on the lowest proposed price, with vendors receiving points proportional to their pricing relative to the lowest bid. Vendors must complete and submit multiple documents, including a Location Certification Form, Pricing Worksheet, Performance Measures Questionnaire, Plan Design Questionnaire, and Vendor Reference Verification Form. The proposal requires detailed information about the vendor's organizational structure, customer service capabilities, implementation approach, and previous experience providing similar services to governmental or public entities of comparable size.
