About this file
This is a reference questionnaire issued by the Nevada State Purchasing Division, Department of Administration, in support of a formal Request for Proposals (RFP) for UPS Service Maintenance at Nevada Department of Motor Vehicle offices. The solicitation number is 81DMV-S3894, and the reference questionnaire is due by June 25, 2026. The contract opportunity involves providing inspections, corrective maintenance, and emergency maintenance services for Uninterruptible Power Supply systems and components located at Department of Motor Vehicle offices in both northern and southern Nevada. Christian Armas serves as the single point of contact for inquiries regarding this solicitation and should receive completed reference questionnaires via email with the solicitation number in the subject line.
The reference questionnaire requires vendors to provide client references from current or former clients for projects of similar size and scope. Vendors must submit completed questionnaires from client representatives who can evaluate their performance across multiple criteria including knowledge and expertise, flexibility relative to changes in scope and timelines, satisfaction with delivered materials and products, schedule adherence, customer service responsiveness, billing accuracy, problem resolution capability, and overall likelihood of future recommendation. Ratings are requested on a scale of 0-10, with detailed guidance provided for performance levels ranging from poor or inadequate (0) to exceptional (10). Responses marked as unknown should be indicated as "U/K" and non-applicable items as "N/A." Completed reference questionnaires are designated as confidential business information and will not be released to the public, allowing for candid client feedback during the vendor evaluation process.
