Instructions
| Form 5: |
| (1) Enter Prime(Offeror) Labor rates in Form 5, add formulas to convert from Offeror Fiscal Year(OFY) to Contract Year (CY) rates. |
| (2) Enter mapping from Schedule of Rates Labor Category to Offeror Internal Labor Category. |
| *Offerors may map multiple Schedule of Rates Labor Categories to the same Offeror Internal Labor Category, as long as the Offeror Internal Labor Category is qualified to perform the work. |
| Examples: |
| Engineers I-V may be mapped to fewer than 5 Internal Engineering Categories as long as the Education and experience minimums are met. |
| A higher qualified labor category can always do the work of a lower level labor category when that labor category is unavailable, and can be mapped that way. |
| If, as an example, an Internal Labor Category of 'Technician' qualifies as a Mechanical Technician AND a Rigger, it can be mapped to both. |
| (3) Enter Prime(Offeror) Indirect rates in Form 5, add formulas to convert from Offeror Fiscal Year(OFY) to Contract Year (CY) rates. |
| Form 4x: |
| (1)Bring Prime(Offeror) Labor and Indirect Rates in Form 5 over to this form and build up burdened rate, excluding profit, assign percentage of effort for labor category to Offeror and SubKs. |
| (2)Enter each Subcontractor Bill Rates (Fully Burdened Rates with fee/profit) in Form 4x, assign percentage for labor category. Make sure each labor category percentage of effort adds to 100%. |
| *Insert lines as necessary to accommodate subcontractors. |
| *Insure that significant subcontractors have been allocated adequate effort to be considered significant in accordance with solicitation. |
| *Make sure Significant Subcontractors submit to NASA the unredacted forms required. |
| (3) Build out all years. Copying year 1, once complete to years 2-10 may be the most efficient method, if mix of effort between prime and subcontractors is projected to be consistent. |
| Form 3x1, 3x2, and so on: |
| (1)Bring SubK Bill Rates in Form 4x over to the appropriate 3xN form and apply any Offeror subcontractor burdens to bill rate. |
| Form 3x: |
| (1)Bring Prime(Offeror) burdened Labor rates(excluding profit) in Form 4x, bring over percentage of effort assigned. |
| Form 2x: |
| (1) Develop formulas to created weighted average rate by labor category based upon percentage of effort(s) provede by Prime and subcontractors. |
| (2) Enter any indirect rates applied to ODCS, by year |
| (3) Enter Firm Fixed Price (FFP) and Cost Plus Fixed Fee (CPFF) Rates. |

Subcontractor profit/fee should be included. Form 2 is already populated with the labor categories Offerors must propose rates for. Offerors shall not add or delete any labor category listed in Form 1, and Offerors shall not propose multiple sets of rates for one labor category (e.g. prime Offeror’s rates for a particular labor category and a subcontractor’s rates for the same labor category). For each labor category for each contract year the following information is required in Form 2: |
i. Identify if the labor category is prime, subcontractor, or a composite (i.e. a blended rate consisting of prime and subcontract labor). If a proposed rate is a composite, Offerors shall explain the calculation of that rate based on the rates proposed in Form 3 and 3a and so on (e.g. providing the specific weightings applied to the prime and subcontractor’s rates from Form 3 and 3a which were used to calculate the proposed composite rate).
ii. The proposed burdened labor rate thru all applicable indirect rates [excluding profit/fee] for each contract year.
iii. The proposed indirect rate(s) for ODCs for each contract year.
iv. The proposed profit rate for FFP task orders.
v. The proposed fee rate for CPFF task orders.
Form 2 shall be submitted by each significant subcontractor [as defined in Section L.15 of the RFP] in its own proposal and shall be submitted directly to NASA no later than the date and time specified in the instructions for receipt of offers for this solicitation. Subcontractors not meeting the thresholds at Section L.15 are not required to provide this Form nor any supporting cost detail (i.e. no cost forms are required); the prime Offeror only needs to provide the fully-burdened rate(s) for these subcontractor(s) and rationale for reasonableness of the fully-burdened rate(s).

If a proposed rate is a composite, Offerors shall explain the calculation of that rate based on the rates proposed in Form 3 and 3a and so on (e.g. providing the specific weightings applied to the prime and subcontractor’s rates from Form 3 and 3a which were used to calculate the proposed composite rate).
