The North American Industry Classification System code is 236220 and size standard is $36.5 million average annual receipts. Offerors should respond with capabilities statements limited to ten pages by February 14, 2019.

80GRC019R0007 Glenn Multiple Award Construction Contracts-2 (GMACC-2): Attachment B Past Performance Questionnaire’s (PPQs) are DUE BY 12 p.m. NOON Eastern on May 29, 2019.

Thank you for your time! Completed PPQ’s are considered source sensitive (FAR 3.104).

1. Past performance questionnaires should be completed by BOTH the Contracting Officer (CO) and Contracting Officer

Representative (COR), which are the individuals most knowledgeable of the contractor's day-to-day operations and overall condition of services being rendered. However, that individual is encouraged to supplement their own knowledge of the contractor's performance with the judgment of others in the organization, as applicable.

2. Responses to this questionnaire shall be sent to tyler.r.braden@nasa.gov and Not returned through the contractor.

PPQs are DUE BY 12 p.m. NOON Eastern on May 29, 2019.

3. The Subject Line of the PPQ email submissions shall contain: 80GRC019R0007 PPQ and the Offeror’s Company Name.

4. The following descriptions should be used as guidance in completing the PPQ:

Performance Ratings: Based on your knowledge of the contract identified below, please provide your assessment of Contractor performance on the questions listed in the PPQ over the past 5 years. You are encouraged to give narrative “REMARKS”, indicating why you answered, especially for those answers that are “Excellent” or “Poor.” The following definitions are offered for your use in assigning a performance rating:

CODE RATING DESCRIPTION

E Excellent Of exceptional merit; exemplary performance in a timely, efficient, and economical manner; very minor (if any) deficiencies with no adverse effect on overall performance.

VG Very Good Very effective performance, fully responsive to contract requirements; contract requirements accomplished in a timely, efficient, and economical manner for the most part; only minor deficiencies.

G Good Effective performance; fully responsive to contract requirements; reportable deficiencies, but with little identifiable effect on overall performance.

F Fair Meets or only slightly exceeds minimum acceptable standards; adequate results; reportable deficiencies with identifiable, but not substantial, effects on overall performance.

P Poor Does not meet minimum acceptable standards in one or more areas; remedial action required in one or more areas, deficiencies in one or more areas which adversely affect overall performance.
