About this file
This is an organizational chart attachment for a Request for Proposals issued by the City of Detroit Office of Contracting and Procurement on behalf of the Construction and Demolition Department for HVAC Services citywide. The RFP seeks qualified respondents to provide HVAC services throughout Detroit for a three-year contract term. Respondents are required to submit organizational charts in portable document format (.pdf) that reflect only the office or unit expected to provide services under this contract, rather than all company locations if the vendor operates across multiple regions, states, or countries. The organizational chart must clearly identify any vacant or unstaffed positions within the structure.
The attachment provides specific instructions for chart presentation, allowing respondents with multiple positions of the same job title to consolidate the information by listing the position title with the total number of positions and indicating vacant positions separately, rather than naming each individual employee. This approach streamlines the submission process for larger vendors with numerous staff members in similar roles, such as service technicians or apprentices. Respondents must submit their organizational charts prior to the RFP deadline and may contact the City with questions regarding this attachment before the specified deadline.

ATTACHMENT B1
REQUEST FOR PROPOSALS – HVAC SERVICES
ORGANIZATIONAL CHART
Please thoroughly read the following instructions and notices before completing this attachment. Interested vendors must ask any questions related to this attachment prior to the deadline identified in this RFP.
Instructions & Notices
The Respondent must provide an organizational chart for the business in a portable document format (.pdf).
If the business entity includes multiple offices/units across several regions/states/countries, the Respondent only needs to submit (an) organizational chart(s) for the office(s)/unit(s) that may provide services under this contract.
For example, 123 HVAC Services solely operates in the State of Michigan. 123 HVAC Services has offices in Detroit, Lansing, Grand Rapids, and Traverse City. However, 123 HVAC Services only expects staff from the Detroit office to provide services under this contract. So, 123 HVAC Services will only need to provide an organizational chart for the Detroit Office.
The Respondent must clearly indicate those positions (if any) in the organizational chart that are vacant/unstaffed.
If a Respondent has multiple positions of the same job title in the organizational tree, the Respondent is NOT required to list all the staff persons under that title. The Respondent may simply list the job/position title and the number of positions under that title. However, the Respondent must indicate the number of vacant positions under that title.
For example, 123 HVAC Services has four Supervisory Technicians who each manage three Service Technicians and their Apprentices. One Service Technician position is vacant and two Apprentice positions are vacant. Therefore, 123 HVAC Services may present that portion of their organizational chart as follows:
Supervisory Technician – X 4
Service Technician – X 12, 1 vacancy
Apprentice – X 12, 2 vacancies
