About this file

This document is an Attachment F for a Physical Sample Evaluation Stage related to the State of Utah Division of Purchasing's Request for Proposal (RFP) for Security Paper for Printing Vital Records. The RFP is structured as a three-stage evaluation process, with this attachment specifically outlining the second stage, which is a pass/fail physical sample review. Vendors who pass the initial technical criteria threshold will be invited to submit physical samples of stock vital record designs, with samples required to be sent within 14 calendar days of invitation. The contract is for a 5-year term, with vendors required to guarantee pricing for the first 12 months, and the procurement is structured as a single award to the vendor who achieves the highest total RFP score across all evaluation stages.

The physical sample submission is critical to the evaluation, with vendors expected to clearly demonstrate and document security features on the sample. Vendors will not be compensated for sample submissions, and the samples become property of the State, with potential destruction after the RFP is awarded unless specifically requested to be returned. No administrative fee is associated with this contract, and the samples must comprehensively showcase the security paper's features in a manner that allows the evaluation committee to fully assess the product's compliance with the specified requirements.

Physical Sample Evaluation Stage Attachment F DS25-3 Security Paper for Printing Vital Records

PURPOSE OF THIS DOCUMENT: The purpose of this document is to give instruction to the vendors who advance to this stage of the Evaluation process.

EVALUATION PROCESS:

This is the 2nd stage of three evaluation stages. The 1st stage is the Technical Criteria Evaluation (Attachment D). Any vendors who receive the minimum technical criteria threshold score as stated in Attachment D will advance to this evaluation stage of the RFP.

This evaluation stage is a pass/fail stage in which a physical sample will be evaluated by the evaluation committee. The evaluation team will determine if the submitted sample meets or exceeds the minimum expectations as outlined in the Scope of Work (Attachment B). Any vendor who passes this stage will advance to the final evaluation stage, the Cost Evaluation. (Attachment C)

VENDOR INSTRUCTIONS:

1. Vendors who advance to this evaluation stage will be asked by the Division of Purchasing to send a physical example of a stock vital record design. Due to the location(s) of the evaluation committee members, vendors may be asked to send up to three (3) identical samples to various addresses.

2. Samples must include a reference to the security features contained on the security paper. It is acceptable for the vendor to write numbers on the sample and describe the corresponding security features on a separate paper, or to describe them on the actual sample. It is at the vendor’s discretion to clearly describe all security features on the sample so that the evaluation committee can clearly detect each security feature and its location on the sample.

3. Samples must arrive within 14 calendar days following the invitation to send a sample from the Division of Purchasing.

4. Vendors will not be compensated or reimbursed for the expenses of sending a sample. Samples become property of the State. The State will ensure destruction of the sample after the RFP is awarded unless it is requested by the vendor to be returned. Returned samples will be at the expense of the vendor. Because of the sensitive nature of the product and expenses the vendor incurs by sending a sample, please do not send samples unless you are specifically invited to do so by the Division of Purchasing.
