About this file

This document is a Cost Sheet Attachment C for Solicitation #DS25-3, issued by the State of Utah Division of Purchasing for Security Paper for Printing Vital Records. The solicitation seeks a single vendor to provide specialized security paper with complex printing requirements, including two distinct quote scenarios: Quote 1 involves 25,000 pages with 5 unique designs requiring front and back printing, and Quote 2 involves 10,000 pages with 1 design. The contract will have a 5-year term, with vendors required to guarantee pricing for the first 12 months. The cost evaluation will assign points based on the lowest total cost per sheet, with a maximum of 300 cost points available that will be combined with technical scores to determine the overall winner.

The pricing structure involves detailed cost components including artwork/design fees, plate/cylinder creation, physical proof, printing costs, packaging, and shipping to multiple zip codes. Vendors must provide comprehensive pricing details covering various paper options and security features, which include specialized requirements such as sequential numbering, barcodes, watermarking, engraving, chemical security, security fibers, and void pantograph capabilities. The evaluation methodology involves calculating a cost per page across the two quote scenarios, with the lowest-priced vendor receiving maximum cost points. Additional non-scored but contractually binding tables require vendors to outline shipping calculations, artwork/setup fees, and potential bulk discount pricing tiers.
