About this file
This document is an RFP Score Sheet for the State of Utah Division of Purchasing, specifically for a solicitation (#DS25-3) to procure Security Paper for Printing Vital Records. The procurement is structured as a single-award contract with a 5-year term, during which the vendor must guarantee pricing for the first 12 months. The evaluation process is multi-staged, requiring vendors to pass minimum mandatory requirements, technical evaluation, and a physical sample stage before advancing to a cost evaluation. Vendors must score at least 65% (455 points) on the technical evaluation to remain competitive, with a total possible score of 1,000 points across technical (70%) and cost (30%) criteria.
The scoring methodology is comprehensive, with technical criteria weighted across four key areas: Experience & Certifications (20%), Paper Features (40%), Ordering Process & Customer Service (50%), and Quality Control & Warranty (30%). The cost evaluation will be calculated using a formula that awards maximum points (300) to the lowest proposed price, with other vendors receiving proportional points. The ultimate goal is to select a single vendor who demonstrates the highest combined technical and cost performance for providing security paper specifically designed for vital records printing.
