About this file
This is a U.S. Coast Guard pre-construction notification checklist for bridge projects in the Jacksonville sector, involving coordination between the Coast Guard's Southeast District Bridge Administration, Sector Jacksonville Waterways, and local bridge owners and contractors in Florida. The checklist establishes mandatory notification and operational requirements for any bridge construction or maintenance project affecting navigable waterways. Contractors must submit 60 days advance notice prior to project start and 30 days advance notice before any channel obstructions, closures, or opening restrictions occur. The form requires detailed project information including roadway and waterway identification, in-water equipment specifications, petroleum or hazardous material storage descriptions, proposed channel restrictions or vertical clearance limitations, and operational procedures for movable spans. For movable span operations, contractors must specify whether they require complete closure, single leaf opening, limited openings, or advance notice protocols, and provide advance notice timeframes for full opening and equipment repositioning to restore channel access.
All notifications must be submitted to Sector Jacksonville Waterways (D07-DG-SECJAX-WATERWAYS@uscg.mil) and Southeast District Bridge Administration (Jennifer.N.Zercher@uscg.mil). The checklist applies to the County of Volusia's Main Street Bridge No. 794004 Repairs project in Daytona Beach, which involves cleaning and painting structural steel on bascule and flacking spans, installing web repair plates, replacing bolts and rivets, removing abandoned air buffers and strike plates, and rehabilitating auxiliary drive machinery bearings. The contractor must maintain channel traffic, implement pollution controls, conduct environmental testing, perform functional span balancing checks, and provide operations and maintenance manuals and as-built surveys upon project completion. Hurricane plans are required for projects occurring between June and November.
