About this file
This document is a U.S. Department of Housing and Urban Development (HUD) Schedule of Change Orders form (HUD-51002), designed for contractors to report details of approved change orders for public and Indian housing projects. The form is part of HUD's contract administration process, used to track modifications to construction or renovation contracts, with a specific focus on ensuring development costs remain consistent with HUD construction requirements.
The form provides a standardized method for documenting change orders, requiring contractors to attach the completed form to periodic payment estimates and submit it to the relevant Public Housing Agency. The document emphasizes the legal importance of accuracy, with a warning that false statements can result in criminal, civil, and administrative sanctions, including potential fines and confinement. The form captures critical contract modification details such as change order numbers, dates, total amounts, and the value of work completed, serving as an official record for tracking project evolution and financial adjustments.
