About this file

This is a Price Schedule attachment for a snow removal Request for Proposal (RFC) issued by Salt Lake County Contracts and Procurement on behalf of Facilities Management, Aging and Adult Services, Salt Lake County Health Department, and the District Attorney's offices. The solicitation seeks snow plowing, snow shoveling, application of ice melt for sidewalks and stairs, and sodium chloride application for parking lots and streets at eight County facility locations: Ellis R. Shipp Public Health Center, Environmental Health Admin, South Main Public Health Center, Magna-Kennecott Senior Center, South Redwood Health Center, Household Hazardous Waste facility, DA Downtown Building, and DA West Jordan Building. Salt Lake County may contract with multiple bidders for snow removal services under this solicitation.

Bidders must submit pricing using the standardized Price Schedule, with an hourly rate applied uniformly across all locations in Column 1, a specific number of hours (not a range) for snow removal of 2 inches or greater accumulation in Column 2, separate pricing for ice melt application and rock salt application in Columns 4 and 5, resulting in a total "Push Price" for each location in Column 6 that serves as the not-to-exceed amount the County will pay per service event. Bid evaluation is based on the Location Total in Column 6, with award made to the lowest responsive bidder for each location. Bidders may elect to bid on all locations or only selected locations of interest. The County reserves the right to reject bids as non-responsive if bidders submit different hourly rates by location or provide range estimates for hours rather than specific quantities.

Bidders are to attach this Bid Schedule with their bid

NOTE:

1. In Column 1, Bidder must bid the same hourly rate for each site. Submission of different hourly rates for any of the locations may result in County rejecting the bid as non-responsive.

2. Bidder will not give a range estimate in Column 2 (for example, 2-3 hours). Submission of a range may result in County rejecting the bid as non-responsive.

3. Column 1 and 2 will be used to calculate Bidder’s price to entirely remove the snow at a location. Column 4 is to apply ice melt at each location. Column 5 is to apply rock salt at each location. The pricing provided per location in Column 6 is the total not to exceed amount County will pay each time Bidder removes the snow at that location, consistent with the Contract.

4. Bid evaluation will be based on the Location Total (column 6). Award will be made to the lowest responsive bidder for each location. Bidders may be awarded one or more locations as determined by the County.

5. Bidder may bid on all locations or may select only the locations bidder is interested in.
