SECTION C – DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK

SCOPE OF WORK

Site Prep for Clinical Modular Building Installation

Project # OK2CAS01H4

March 12, 2025

I. Objective:

The objective of this project is to prepare the site location for installation of a new five‐section 64’8” x 72’ modular building for clinical use remotely located from other buildings at the US PHS Indian Hospital located in Claremore, Oklahoma. The hospital is a 110,000+ ft2 hospital building that consists of outpatient services such as OB/GYN, Behavioral Health, Pediatrics, Medical/Urgent Clinics, Contract Health, Dental, WIC, Pharmacy, Diabetes Education, and Public Health, as well as in‐patient services that includes ER, OB, in‐patient, OR, and ICU.

II. Background:

A. A 64’8” x 72’ modular building was purchased for COVID use (but has not been constructed) and is awaiting site preparations to be performed for delivery of the unit. The modular building consists of five modular sections that will be connected on site along with utilities between the five units. The interior of the building consists of four main hallways and thirty rooms described as follows: three ADA‐compliant restrooms, one waiting room, one registration office, one janitor’s closet, one breakroom, one soiled holding room, three storage rooms (one general, one clean/sterile, one medical supplies), one procedure room, two staff work rooms, eight screening rooms, and eight exam rooms. Refer to Attachment J02‐A. Modular Building Plans for details and Attachment J02‐B. Site Conditions for existing site view with utilities.

1. The building water and sewer utility connections are available from the main building crawlspace on the south side of the install site under Pediatrics Department. Electric service is located to the southwest of the proposed building location at the administration building’s 240V, 1‐phase electrical transformer on the west end of the building. Electrical service from this location will be trenched to the proposal location for the new modular building’s 240V 1‐phase electrical service. The transformer will be replaced and upsized to accommodate both the existing administration building and the new clinical modular building. Other utilities and systems shall come from the main or administration building (fire alarm system, IT network, etc.).

2. The five modular sections will be brought in from the north and east via main parking lot asphaltic driveway and craned into position. Contractor to provide crane services. Hitches on all sections will be on the north side, removed by building delivery personnel during install.

3. The modular building will be delivered with no final exterior surface installed with contractors to construct a lower brick wall on concrete footings and EIFS applied to all exterior walls. A new metal‐covered roof with roof extensions to the east and west will be added to cover the new building, front entrance area, and rear entrance area. Exterior will match existing administration building exterior in color, brick, EIFS, and roof pitch.

4. The modular building will be set in place on concrete footings and foundations with floor level 3’ above grade with planned landings, stairs, ramps, and covered roofs over the front and rear entrance area with access to the front via sidewalk and parking lot facing east.

Sidewalk re‐configuration is required to address access to the building and relocation of existing sidewalks.

5. Temporary steps will be installed by contractor during installation until final steps, landing, and ramps are installed.

6. The site location for the modular building is located on the north section of the hospital campus in front of the Pediatrics location in the main building with the administration building adjacent to the west. This area is currently campus grounds where no previous buildings existed.

III. Scope of Work: Reference Attachments J02‐A through H for All Work
