About this file
This document contains a past performance evaluation questionnaire to be completed by references for federal contractors bidding on an upcoming project. The questionnaire requests ratings and comments on timeliness, quality of work, business relations, cost control, and management of key personnel on previous projects. Details are solicited on contract type, period of performance, amounts, completion dates and any performance issues. Overall satisfaction and willingness to work with the contractor again are also addressed. The related pre-solicitation notice indicates the Department of Health and Human Services Food and Drug Administration Office of Acquisition and Grant Services will be soliciting proposals for a project to construct a new chiller plant and associated building improvements. No other requirement details are provided at this time beyond the solicitation number and project name.
