About this file
This is a Quality Assurance Surveillance Plan (QASP) template for a Department of Homeland Security (DHS) contract involving the configuration upfit and Operations & Maintenance (O&M) service support for four Boeing 737-700 aircraft. The document is designated as Attachment 3 to the contract and serves as a living document subject to revision with contractor coordination.
The QASP establishes the framework for government oversight of contractor performance by defining what will be monitored, how monitoring will occur, who will conduct surveillance, and how results will be documented. The primary performance expectation is completion of upfitting configuration for four Boeing 737-700 aircraft and return of the aircraft to airworthy status during the O&M service support period. The Contracting Officer is S. Andrew Whittaker (DHS-HQ, 202.984.0251, stephen.whittaker@hq.dhs.gov), while the Contracting Officer's Representative position is to be determined. Surveillance methods include direct observation, periodic inspection, user surveys, validated customer complaints, progress meetings, and analysis of contractor progress reports. Performance ratings are categorized as Exceptional (exceeds requirements), Satisfactory (meets requirements), or Unsatisfactory (does not meet requirements). The COR shall meet quarterly with the contractor to assess performance and provide written assessments. Acceptable performance is documented through Performance Reports, while unacceptable performance triggers a Contract Discrepancy Report (CDR) requiring the contractor to submit a corrective action plan. The sample QASP table identifies 100% compliance with all DHS-specific PWS deliverable requirements as a key performance standard, with remedial work at contractor expense as the negative incentive for non-compliance. No positive incentives are provided under this contract.

Contract Number:
Requirement Description: 737-700s Aircraft Configuration Upfit and Operations &
Maintenance (O&M) Service Support Contractor’s Name:
I. PURPOSE
This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains / answers the following:
• What will be monitored.
• How monitoring will take place.
• Who will conduct the monitoring?
• How monitoring efforts and results will be documented.
This QASP does not detail how the contractor accomplishes the work. Rather, the QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance.
This QASP is a “living document” and the Government may review and revise it on a regular basis.
However, the Government shall coordinate changes with the contractor. Copies of the original QASP and revisions shall be provided to the contractor and Government officials implementing surveillance activities.
The Contractor is responsible for the quality of all work performed under this contract. The Contractor’s QCP sets forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the PWS. This QASP details how and when the government will survey, observe, test, sample, evaluate, and document Contractor performance.
The following FAR clauses may apply depending on contract type:
52.246-4 Inspection of Services – Fixed-Price Unit Price (FFUP)

V. INCENTIVES
The Government have no intention of providing incentives.
VI. METHODS OF QUALITY ASSURANCE
Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the administration of this QASP.
Regardless of the surveillance method, the COR shall always contact the Contractor's task manager or on-site representative when an issue is identified and inform the manager of the specifics of the problem. The COR, with assistance from the CO, if needed, shall be responsible for monitoring the contractor’s performance in meeting a specific performance standard/AQL.
Direct Observation. (Can be performed periodically or through 100% surveillance.)
Periodic Inspection. (Uses a comprehensive evaluation of selected outputs. Inspections may be scheduled [Daily, Weekly, Monthly, Quarterly, or annually] or unscheduled, as required.)
User Survey Validated User/Customer Complaints Progress or Status Meetings Analysis of Contractor's Progress Reports. (Evaluate cost, schedule, etc.)
Surveillance results may be used in the basis for actions (to include performance ratings) against the Contractor. In such cases, the Inspection of Services clause in the contract becomes the basis for the CO’s actions.

The CDR will specify if the Contractor is required to prepare a corrective action plan to document how the contractor shall correct the unacceptable performance and avoid a recurrence. The CDR will also state how long after receipt the contractor has to present this corrective action plan to the COR. The Government shall review the Contractor's corrective action plan to determine acceptability.
Any CDRs may become a part of the supporting documentation for contract payment deductions, fixed fee deductions, award fee nonpayment, or other contractual actions deemed necessary by the
CO.

During contract order performance the COR will periodically analyze whether the negotiated frequency of surveillance is appropriate for the work being performed.
