About this file
This is an evaluation scoresheet for Attachment D of Request for Proposals WK2706-CNS issued by the Salt Lake City School District (SLCSD) for classroom nursing services. The SLCSD seeks qualified nursing agencies capable of providing licensed registered nurses (RNs) to deliver health care services to students with complex health care needs within district classrooms. The evaluation scoresheet establishes the framework for assessing competitive sealed proposals from qualified vendors, with all submissions required through the Bonfire system and subject to a deadline after which late submissions will be rejected.
The evaluation methodology consists of two primary components: technical criteria weighted at 70 percent of total points (700 points possible) and price criteria weighted at 30 percent (300 points possible), for a combined total evaluation of 1,000 points. Technical scoring utilizes a 0-5 scale across nine weighted criteria categories, with a mandatory minimum threshold of 70 percent (490 points) required for technical qualification. Scoreable technical criteria include the agency's five years of nurse staffing service experience, detailed narratives addressing backup/substitute plans, hiring requirements and OIG exclusion list checking procedures, new student placement timelines, work assessment, staff history and experience, experience with schools and pediatric patients, clinical supervisory staff qualifications, and Utah-based physical office and supervisory staff locations. Price evaluation focuses on hourly rates for RN and LPN services. Mandatory requirements include vendor agreements to ensure Medicaid compliance, maintenance of nursing licenses and validations, FERPA compliance, staff proficiency, medical malpractice insurance, CMS-recognized accreditation or state survey certification, Utah business licensing, and availability for oral presentation. All proposals must demonstrate that nursing staff reside within 50 miles of Salt Lake City, Utah.
