About this file
This is a Drug Free Workplace Certification form required by the Town of Palm Beach, Florida for the Sea Turtle Nesting Monitoring contract opportunity. The certification mandates that the selected contractor establish and maintain a drug-free workplace by publishing a statement prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances; establishing a drug-free awareness program that educates employees about dangers of drug abuse, the grantee's policy, available counseling and rehabilitation programs, and penalties for violations; requiring each employee engaged in grant performance to receive a copy of the prohibition statement; notifying employees that compliance with the drug-free workplace policy is a condition of employment; and requiring employees to report any criminal drug statute convictions occurring in the workplace within five days of conviction. The contractor must notify the Town of Palm Beach within ten days of receiving notice of any employee conviction and must take appropriate personnel action, up to and including termination, or require the employee to participate in an approved drug abuse assistance or rehabilitation program within thirty days of receiving notice.
The certifying contractor must demonstrate good faith effort to maintain a drug-free workplace through continuous implementation of all specified requirements. The form requires signature and authorization by an authorized agent of the firm, including the firm name, date, agent name and title, email address, phone number, and signature or electronic signature. This certification is a standard compliance requirement for contractors performing work on behalf of the Town of Palm Beach and must be completed and submitted as part of the proposal package through the Town's Bonfire eProcurement platform.
