About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters document required for the Building Automation Systems – Maintenance and Repair Services contract opportunity with the City of North Carolina's General Services Department, Facility Operations Division. The document applies to bidders, contractors, and subcontractors responding to the Request for Proposal for Carrier-based building automation systems maintenance, repair, and support services for the City's portfolio of approximately 180 buildings, including office facilities, fire houses, police division offices, equipment shops, and cultural venues such as museums and arenas.
The certification requires bidders and their principals—including officers, directors, managers, and entities holding greater than 10 percent equity interest—to attest that they are not presently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal or state agency. Bidders must also confirm that within the three-year period preceding the proposal, neither they nor their principals have been convicted of or had civil judgments rendered against them for fraud, criminal offenses related to public transactions, anti-trust violations, procurement statute violations, embezzlement, theft, forgery, bribery, falsification of records, false statements, or receipt of stolen property. Additionally, bidders certify they are not presently indicted or criminally or civilly charged with any such offenses and have not had public transactions terminated for cause or default within three years. False certification may result in proposal rejection, contract termination, or criminal prosecution. Bidders unable to certify to these statements must provide a written explanation and check the applicable box on the form.
