About this file

This is an RFP evaluation scoresheet for the State of Utah Division of Purchasing's procurement of portable toilets and services (MH26-9) across five regions of the state. The procurement may result in multiple contract awards based on vendors' ability to meet technical and overall scoring thresholds. The contract term is five years, with vendors required to guarantee pricing for a minimum of one year.

The evaluation scoresheet establishes a 1,000-point total scoring system comprised of 80 percent technical evaluation (800 points) and 20 percent cost evaluation (200 points). Technical scoring is divided into four criteria: Equipment Specifications and Inventory (45 points, 28.1 percent weight), Service and Maintenance Quality (40 points, 25.0 percent weight), Operational Response and Compliance (40 points, 25.0 percent weight), and Experience and Account Management (35 points, 21.9 percent weight). Vendors must achieve a minimum of 520 technical points and a combined total of 620 points to be eligible for award. Cost points are allocated based on a formula where the lowest proposed price receives 100 percent of available points, with other proposals receiving proportional credit based on their price ratio to the lowest bid. An administrative fee of 0.25 percent applies to the contract. Mandatory prerequisites for evaluation include submission of Attachment C (Cost Sheet) and Attachment D (Technical Criteria Responses).

* Purchasing will use the following cost formula: The points assigned to each Vendor's cost proposal will be based on the lowest proposal price (200 Points per Region and there are 5 Regions). The Vendor with the lowest Proposed Price of each Region will receive 100% of the price points for that individual Region. All other Vendors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price).
