About this file

This is a cost sheet attachment for a Request for Proposal issued by the State of Utah Division of Purchasing for portable toilet services across five regions of the state. The RFP seeks multiple contracts to provide full-service portable toilets with hand sanitizer over a five-year contract term. Vendors are required to submit pricing proposals for at least one region but may serve any or all five regions. The cost proposal must include three service rate categories: monthly service rates, weekly service rates, and per-delivery service fees. Vendors must not add or remove lines from the cost proposal spreadsheet, as doing so may result in non-responsiveness designation. Full-service requirements mandate that vendors perform evacuation (complete pump-out of waste tanks), sanitization (thorough chemical wash of interior surfaces using hospital-grade disinfectants), deodorization (recharging tanks with fresh chemical and anti-bacterial solution), and restocking (replenishment of hand sanitizer and paper products) during each scheduled visit.

Cost evaluation will be calculated using a specific formula whereby the vendor with the lowest proposed price in each region receives 100 percent of available price points for that region, with other vendors receiving points proportionally based on how their pricing compares to the lowest bid. Vendors may propose additional services and products such as ADA-accessible wheelchair units and must provide catalog information with associated pricing. A rural service surcharge applies to assignments located more than 120 miles one way from the vendor's point of hire, with specific rates to be mutually agreed upon in writing before service commencement. Vendors must guarantee pricing for one year, and an administrative fee of 0.25 percent applies to the contract. The cost sheet provides space for vendors to define response timeframes for additional unit requests and specify any additional rates or services offered outside the standard proposal categories.

Purchasing will use the following cost formula: The points assigned to each Vendor's cost proposal will be based on the lowest proposal price (200 Points per Region and there are 5 Regions). The Vendor with the lowest Proposed Price of each Region will receive 100% of the price points for that individual Region. All other Vendors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. The formula to compute the points is: Cost Points x (Lowest Proposed Price/Proposed Price).
To qualify as "Full Service," the Vendor must perform the following during each scheduled visit:

1. Evacuation: Complete pump-out of the waste tank.

2. Sanitization: Thorough "top-to-bottom" chemical wash of all interior surfaces (walls, floor, seat, and urinals) using hospital-grade disinfectants.

3. Deodorization: Recharging the tank with fresh chemical deodorizer and anti-bacterial solution.

4. Restocking: Replenishment of hand sanitizer and all paper products.
