About this file

This is a Cost Sheet Attachment for a statewide Disaster Management and Recovery Consultants contract opportunity issued by the State of Utah Division of Purchasing. The RFP seeks to establish a contract with one or more vendors to provide disaster management and recovery consulting services, including support for FEMA Public Assistance programs, hazard mitigation, federal disaster recovery initiatives, and related technical assistance. The contract has a term of five years, with vendors required to guarantee pricing for a minimum of one year. The RFP will result in multiple awards based on a combined scoring methodology where vendors must achieve at least 70% of available points to qualify for a statewide cooperative contract. Vendors must first attain a minimum of 70% in technical criteria points (490 of 700 available points) to advance to the cost evaluation stage.

The cost evaluation is based on average hourly rates across five job classifications: Principal Consultant, Senior Consultant, Consultant, Junior Consultant, and Program and Administrative Support staff. All yellow cells in the cost sheet must be completed with specific hourly rates; incomplete submissions, price ranges, or zero values will render proposals non-responsive. The vendor with the most competitive average hourly rate receives 100% of the 300 available cost points, with all other vendors receiving a proportional share calculated using the formula: Cost Points × (Most Competitive Average Hourly Rate / Proposed Average Hourly Rate). Travel costs are excluded from hourly rates and are reimbursed separately at state rates if approved by the Eligible User. The RFP carries an administrative fee of 0%, and vendors must submit cost sheet questions or clarification requests during the designated Q&A period; failure to do so constitutes acceptance of all pricing requirements.
