About this file
This file contains Schedule A and Schedule B certification and company profile forms for the Alpine School District's Request for Proposal (RFP) for Fire Alarm Services, issued by Alpine School District's Department of Purchasing with the conducting procurement unit being the Physical Facilities Department. The RFP seeks a qualified vendor to supply parts, labor, and design services to retrofit, replace, repair, or add to existing fire alarm systems throughout the district. The proposal closing date and time is Thursday, February 5, 2026, at 2:00 PM Mountain Time. The resulting contract will have a maximum term of five years, with the understanding that Alpine School District will be reconfigured effective July 1, 2027, into three separate districts (Lake Mountain, Aspen Peaks, and Timpanogos School Districts), and any awarded vendor must honor pricing and allow these new districts to enter into agreements under the same terms and duration.
Schedule B requires proposing vendors to provide comprehensive company information including company history, employee count, qualifications, years in business, service department details with technician experience and response times, past experience with Alpine School District or similar organizations with descriptions of work performed and contact information, product lead times and turnaround times, warranty information for both parts and labor, and a minimum of three references from similar service contracts. Vendors must also disclose any categories or items in the RFP for which they are not qualified or licensed to perform. All proposals must be accompanied by a completed Schedule A certification form wherein the authorized company representative certifies they have read and understand the RFP, possess adequate personnel and resources, are properly licensed to conduct business in Utah, and confirm no prior contact or negotiations have occurred regarding the proposal.
