About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division, located at 101 Camino Entrada, Building 3, Los Alamos, NM 87544. The form is a standard certification document that requires potential contractors or bidders to disclose and certify their legal and ethical standing for participating in county transactions. The certification requires the offeror to attest that they are not currently debarred, suspended, or excluded from public transactions, have not been convicted of fraud or other criminal offenses within the past three years, and are not current or former county employees or immediate family members of county employees or public officials.

The document outlines specific conditions that would disqualify an offeror, including convictions related to fraud, antitrust violations, embezzlement, theft, forgery, bribery, and other offenses indicating a lack of business integrity. The form emphasizes that any falsification of statements will result in non-consideration of the proposal or potential rescission of a contract award. While the form does not specify a particular procurement opportunity, it serves as a mandatory compliance document to ensure the ethical and legal reliability of potential contractors doing business with Los Alamos County.
