About this file

This is a Disclosure of Investment Activities in Iran certification form issued by the County of Bergen Division of Purchasing for the Roadway Repair Materials cooperative purchasing contract (BC-BID-26-26). The form requires all bidders to certify compliance with New Jersey Public Law 2012, c. 25, which prohibits investment activities in Iran. Bidders must attest under penalty of perjury that neither they nor any of their parents, subsidiaries, or affiliates are listed on the New Jersey Department of Treasury's Chapter 25 list of entities engaged in prohibited activities in Iran. The certification must be completed prior to bid submission; failure to complete the form will render a proposal non-responsive. The contract is a twelve-month agreement commencing July 1, 2025 through June 30, 2026, and will supply various roadway repair materials including bituminous concrete, roadway patch materials, catch basin components, manhole frames and covers, asphalt solvents, roadway plates, masonry materials, and precast concrete inlets and trenches for the County of Bergen Department of Public Works and registered New Jersey Cooperative Purchasing Alliance members.

Bidders must review the Department of Treasury's Chapter 25 list available on the state website prior to certification. The form provides two certification options: bidders may certify full compliance and proceed directly to signature, or bidders unable to certify compliance must provide detailed descriptions of their investment activities in Iran in Part 2. All certifications require the signatory to acknowledge that false statements constitute criminal offense and material breach of contract, and that the State of New Jersey may declare any resulting contracts void and unenforceable. Signatories also acknowledge a continuing obligation to notify the State in writing of any changes to the certified information through contract completion.
