About this file
This document is the Proposer Information Sheet for the "BHWI Clinical Supervision Expansion Program Round 2" request for grant proposal (RFGP) issued by the state of Oregon. The RFGP seeks proposals from organizations to provide behavioral health care services that increase supervisory capacity and advance health equity in culturally and linguistically specific communities in Marion County and the city of Salem.
The RFGP requires proposers to provide information about their organization, such as mission, vision, service area, and experience providing behavioral health services. Proposers must also address how the grant funding will be used to increase health equity outcomes, describe the program narrative and key personnel, and outline the evaluation, sustainability, and budget plans. The RFGP has specific word count limits for various sections of the proposal.
