GENERAL REQUIREMENTS

GENERAL

SAFETY REQUIREMENTS

Refer to section 01 35 26, SAFETY REQUIREMENTS for safety and infection control requirements.

GENERAL INTENTION

Contractor shall be completely familiar with the site and furnish labor and materials and perform work for Renovate Various Interior Spaces, JACC as required by drawings and specifications.

Visits to the site by Bidders may be made only by appointment with the Contracting Officer’s Representative (COR).

Prime Contractor Personal Minimum Requirements:

As Required in other parts of the Project Manual and Contract Requirements the following are minimum requirements:

The Superintendent shall be an employee of the Prime Contractor with authority to act as the contractor, responsible for the day-to-day operations at the construction site, planning, quality control, verification of delivery of approved material and equipment, subcontractor coordination responsibilities and execution of the complete project as per the contract documents.

The superintendent shall provide proof to the COR regarding completion of 30 hour OSHA.

Superintendent assigned to this project shall not be assigned to any other project and shall only have duties on one project at a time.

The Superintendent shall sign-in at the beginning of each day of work and Sign-out at the end of the work day at the Office of Engineering Service. No work shall be performed by any contractors or subcontractors without the presence of the superintendent at the actual work site.

The Superintendent shall be responsible for submitting daily work logs to the COR at the end of every work day.

The Prime contractor shall provide services of a Project Manager whose responsibilities would include but not be limited to providing support to the Superintendent at the job site and shall be responsible for finance related tasks and scheduling.

Electrical Work Permit The contractor shall complete the VA Electrical Work Permit with the COR and submit to the COR a minimum of 72 hours prior to start of electrical work within the facility.

Welding and Cutting A daily Hot Work Permit is required and shall be coordinated with the COR. See Section 01 35 26, SAFETY REQUIREMENTS for additional requirements.

Contract Work Hours Normal working hours are 700 am to 4:30 pm. Contractors who need to work other than “normal working hours” for work to be done during those hours must submit a written request to the Contracting Officer for approval. The request must state that the Contractor is requesting to work other than normal working hours at no additional expense to the Government than that which was bid to complete the work. Such requests shall be coordinated with the COR and must be submitted to the CO five (5) working days in advance.

For certain aspects of work the Contractor shall be required to work off duty hours to perform that part of the work, for continued operations of the Medical Center. After duty hours and work shall be coordinated with and approved by the COR prior to commencement of work and must be submitted to the CO five (5) working days in advance.

Noisy demolition work, work at building entrances, electrical or other utility work necessary to pass within, above, traverse hallway corridors, temporary shutdowns of utilities for tie-in, hanging heavy steel, crane lifts, or other hazardous work over the top of occupied areas.

Contractor shall perform work in accordance with requirements of “ASHE Construction in a Healthcare Facility”.

Contractor shall provide Infection Control Plan as required by Memorandum 111-40, Subj: VHA Directive 7715 Safety and Health During Construction dated April 6, 2017. Submit to COR for approval prior to the start of work.

Prior to commencing work, general contractor shall provide proof that an OSHA designated “competent person” (CP) (29 CFR 1926.20(b)(2) will maintain a presence at the work site whenever the general or subcontractors are present. CP will have authority letter.

Per VA Directive 1085, smoking, vaping and smokeless tobacco are prohibited on the grounds of VA facilities, including in vehicles. This Directive applies to all Prime Contractor and Subcontractor employees.

STATEMENT OF BID ITEM(S)

Furnish all labor, tools, materials, equipment, and supervision necessary to perform all work associated with the installation of Project No. 520-23-106, Joint Ambulatory Care Center (JACC)- Pensacola, FL.

The intent of this project to renovate areas in the JACC such as waiting areas, reception areas, changing rooms, and existing office spaces to accommodate space needs for multiple specialties. Contractor to request clarification from COR in writing for any discrepancies between SOW, plans, or specs before beginning work. Certification and accreditation (C&A) requirements per VA Handbook 6500.3 do not apply, and a Security Accreditation is not required.

The new work required is listed as follows:

DEMOLITION

• Contractor to demo existing space as necessary to accommodate new space layout per plans

• Demolition work includes but is not limited to: flooring, doors, frames, trim, casework, ceiling, walls, utilities (electrical-data-plumbing), conduit, lighting, HVAC, fixtures, etc..

• Any piping, ductwork, systems, etc. to be permanently abandoned overhead, in wall or floor slabs, shall be sealed, capped, and tested, before sealing or covering in new walls or back-filled in slabs.

• Contractor to remove existing furniture. See Furniture section for further details.

• Contractor shall not store or stage any materials, demolished or new, in existing main corridors, adjacent rooms, or any other areas, including tools, equipment, ladders, etc. unless coordinated and approved by VA COR.

• Contractor to remove any abandoned, surface-mounted, or exposed electrical conduits, cable ducting, junction boxes, and panels from the site that hinder the production of the work and are not required for the work.

• Contractor to remove all existing misc. wall and ceiling mounted items as directed by COR and safely store items contractor provided storage. Contractor is to reinstall all items at the completion of the project.

1st Floor Charlie-Primary Care Reception

Room 1C177

· Remove existing furniture, artwork, and signage

· Remove the existing suspended ceilings as needed. Existing grid and tiles can be reused as able when reworking ceiling to new wall layout. Replace grid and tiles as needed to ensure like-new appearance.

· Remove existing light fixtures and accessories

· Remove existing supply, return, and exhaust diffusers

· Remove existing floor covering and adhesive

· Remove existing wall base, adhesive, and wall/corner guards

· Remove electrical outlets, switches, building automation devices, and cover plates not able to be reused by new layout.

· Remove partitions in preparation of new work

Room 1C175

· Remove tube delivery system and surrounding chase. Relocate any existing electrical conduits as needed. See Pneumatic Tube System Station Modifications section for further tube system requirements.

· Partially demo existing column furring as needed

· Remove gypsum board furr-downs and prep for new acoustic ceiling and partition construction.

VAV Installations

· Furnish and install new VAVs, hot water piping, ductwork modifications, BAS programming, and commissioning.

· Each new VAV shall be provided with t-stat. All controls installed or replaced shall be native BACnet

· Furnish and install reheat piping and valves from nearest existing piping to the new VAVs. Provide isolation valves for each location supply and return.

· Utilize existing ductwork to feed new and route as needed.

· Insulate all reheat piping and ductwork, match existing materials and thermal

· resistance.

· All duct and insulation seams shall be aluminum taped and coated with insulating

· mastic.

· Insulation of piping and ductwork shall include insulation of appurtenances such as valves, flanges, elbows, tees, circuit setters, dampers, etc.

· Flex duct shall be a maximum of four feet in length and shall only be used to connect supply grills at the end of a duct run.

· All ductwork shall meet or exceed SMACNA standards.

· Maximum flex duct length is 5’

· Demolition will involve removal of unneeded ductwork after new VAVs are installed

· Each location must be completed, inspected and operational before acceptance.

· Terminate all wiring in field panels, build database points, write PPCL code to

· monitor and modulate VAV to control individual environments.

· Provide new field panels and support equipment in areas where panels are at capacity.

· Includes all additional electrical, wire, panel, graphics and PPCL needed to tie into the Siemens System.

· The existing controls system was built and is maintained under contract with Siemens. Therefore, all controls design, installation, and programming shall be through Siemens.

· Provide commissioning for VAVs, include demonstration, and verify operation in all modes

· Provide VAV boxes per VAV schedule in plans. Submit for approval.

· VAVs to be manufacturers standard to include stainless steel supply and return hose kits, access panels, strainers, circuit setters, isolation valves, unions, flex connectors, prewired for t-stat, humidistat, and control program to initiate system dehumidification program.

· Return wall finishes and ceiling grid back to prior construction state

All work listed above shall directly align with the procurement Scope of Work. Any questions or clarifications shall be directed to VA COR prior to bidding and/or construction. Refer to Scope of Work, Graphical Drawings and Specifications for additional information and requirements.

SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR

Drawings and contract documents may be obtained from the website where the solicitation is posted. Additional copies will be at Contractor's expense.

Refer to specifications for installation requirements for each Division. Refer to drawings for estimated quantities and schematic layouts/locations. Contractor shall provide a complete, commissioned, functioning systems. Contractor shall provide all testing reports prior to the start of commissioning. This project will require night and weekend work, Renovation includes requirements for Testing and Balancing the HVAC for the existing and new systems configuration.

Refer to 09 06 00 SCHEDULE OF FINISHES for additional requirements and basis of design tables.

CONSTRUCTION SECURITY REQUIREMENTS

Security Plan:

The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.

The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.

Security Procedures:

General Contractor’s employees shall not enter the project site without appropriate badge. They may also be subject to inspection of their personal effects when entering or leaving the project site.

All employees of general contractor and subcontractors shall comply with VA security management program and obtain permission of the VA police, be identified by project and employer, and restricted from unauthorized access.

Before starting work the General Contractor shall give one week’s notice to the Contracting Officer so that escort arrangements can be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section.

No photography of VA premises is allowed without written permission of the Contracting Officer. Patients and staff are not to be photographed at any time.

VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the premises in the event of a national emergency. The General Contractor may return to the site only with the written approval of theCl Contracting Officer.

Key Control:

The General Contractor shall provide duplicate keys and lock combinations to the Contracting officers representative (COR) for the purpose of security inspections of every area of project including tool boxes and parked machines and take any emergency action.

The General Contractor shall turn over all permanent lock cylinders to the VA locksmith for permanent installation. See Section 08 71 00, DOOR HARDWARE and coordinate.

Document Control:

The General Contractor is responsible for safekeeping of all drawings, project manual and other project information. This information shall be shared only with those with a specific need to accomplish the project.

Certain documents, sketches, videos or photographs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”. Secure such information in separate containers and limit the access to only those who will need it for the project. Return the information to the Contracting Officer upon request.

These security documents shall not be removed or transmitted from the project site without the written approval of Contracting Officer.

All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.

Notify Contracting Officer and Site Security Officer immediately when there is a loss or compromise of “sensitive information”.

All electronic information shall be stored in specified location following VA standards and procedures using an Engineering Document Management Software (EDMS).

Security, access and maintenance of all project drawings, both scanned and electronic shall be performed and tracked through the EDMS system.

“Sensitive information” including drawings and other documents may be attached to e-mail provided all VA encryption procedures are followed.

OPERATIONS AND STORAGE AREAS (FAR 52.236-10)

The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.

Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor at its expense upon completion of the work. With the written consent of the Contracting Officer, the buildings and utilities may be abandoned and need not be removed.

The Contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.

Working space and space available for storing materials shall be as shown on the drawings.

Workers are subject to rules of the Joint Ambulatory Care Center (JACC) applicable to their conduct.

Execute work in such a manner as to interfere as little as possible with work being done by others. Keep roads clear of construction materials, debris, standing construction equipment and vehicles at all times.

Execute work so as to interfere as little as possible with normal functioning of the Care Center as a whole, including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during construction, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required by limited working space.

Do not store materials and equipment in other than assigned areas.

Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by Department of Veterans Affairs in quantities sufficient for not more than two work days. Provide unobstructed access to Care Center areas required to remain in operation.

Where access by VA personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and safety requirements.

Work on areas outside the building and/or construction site during any given phase shall require off hours work, nights and weekends, to maintain continued operations of the JACC Medical Center. Work affecting areas outside the designated building and/or construction site (including bringing materials onsite) shall be scheduled with COR per 1.6 and shall be performed after normal work shifts including nights and weekends.

The work will have to be carefully planned and phased for minimal disruption to the JACC Facility activities. Under no circumstances will any area of the building be shut down and all services must remain operational. Under no circumstances shall there be utility outages, including brief cutovers, anywhere on the campus without the express written consent of the COR. The contractor shall plan and forecast upcoming work which may disrupt normal operations or interfere with staff or patients and report these in the weekly meetings.

Contractor shall provide temporary services, including temporary Air Handlers, Generators, etc.

See Construction drawings for temporary equipment location related to ahu-10.

It may be necessary to postpone or reschedule work to accommodate the needs of the Medical Center. All such time shall be included in the base bid and shall not be billable to the government.

Utilities Services: Where necessary to cut existing pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (except telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by COR. All such actions shall be coordinated with the COR or Utility Company involved:

Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.

Phasing:

THIS PROJECT SHALL BE COMPLETED IN PHASES TO ENSURE LEAST AMOUNT OF DISRUPTION TO VETERANS AND STAFF AND TO ENSURE LIFE SAFETY REQUIREMENTS REMAIN WITHIN REQUIRED LIMITS THROUGHOUT THE ENTIRE CONSTRUCTION PROCESS.

TWO PHASES ARE RECOMMENDED FOR THE WORK:

PHASE 1 TO INCLUDE:

• FIRST FLOOR - BRAVO AREA (PRIMARY CARE)

• SECOND FLOOR - LIMA AREA (EYE CLINIC)

• SECOND FLOOR - INDIA (VACCINE CLINIC)

• SECOND FLOOR - OSCAR (SURGERY TEAM ROOM 2D164-2D165)

PHASE 2 TO INCLUDE:

• FIRST FLOOR - CHARLIE AREA (PRIMARY CARE)

• SECOND FLOOR - OSCAR (SURGERY TEAM ROOM 2F101-2F102)

AT CONTRACTOR'S DISCRETION:

• FIRST FLOOR - FOXTROT - NEW APC CLOSET

The Medical Center must maintain its ongoing operation. Therefore, any interruption in service must be scheduled and coordinated with the COR to ensure that no lapses in operation occur. It is the CONTRACTOR'S responsibility to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, the interim life safety measure to be used during the work, and a schedule defining the duration of the work with milestone subtasks. The work to be outlined shall include, but not be limited to:

To ensure such executions, Contractor shall furnish the COR with a schedule of approximate dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. In addition, Contractor shall notify the COR two weeks in advance of the proposed date of starting work in each specific area of site, building or portion thereof. Arrange such dates to ensure accomplishment of this work in successive phases mutually agreeable to COR and Contractor.

Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Centers operations will not be hindered. Contractor shall permit access to Department of Veterans Affairs personnel and patients through other construction areas which serve as routes of access to such affected areas and equipment. These routes whether access or egress shall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and staff access. Coordinate alteration work in areas occupied by Department of Veterans Affairs so that Medical Center operations will continue during the construction period.

Immediate areas of alterations not mentioned in preceding Subparagraph 1 will be temporarily vacated while alterations are performed.

Utilities Services: Maintain existing utility services for JACC Facility at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (including telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by COR.

No utility service such as water, gas, steam, sewers or electricity, or fire protection systems and communications systems may be interrupted without prior approval of COR. Electrical work shall be accomplished with all affected circuits or equipment de-energized. When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without a detailed work plan, the Medical Center Director’s prior knowledge and written approval. Refer to specification Sections 26 05 11, REQUIREMENTS FOR ELECTRICAL INSTALLATIONS, 27 05 11 REQUIREMENTS FOR COMMUNICATIONS INSTALLATIONS and 28 05 00, COMMON WORK RESULTS FOR ELECTRONIC SAFETY AND SECURITY for additional requirements.

Contractor shall submit a request to interrupt any such services to COR, in writing, 2 weeks in advance of proposed interruption. Request shall state reason, date, exact time of, and approximate duration of such interruption.

Contractor will be advised (in writing) of approval of request, or of which other date and/or time such interruption will cause least inconvenience to operations of the JACC Facility. Interruption time approved by COR may occur at other than Contractor's normal working hours.

Interruptions such as utility interruptions/ closures of any system must be requested, in writing, at least 2 weeks prior to the desired time and shall be performed as directed by the COR .

In case of a contract construction emergency, service will be interrupted on approval of COR . Such approval will be confirmed in writing as soon as practical.

Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.

Abandoned Lines: All service lines such as wires, cables, conduits, ducts, pipes and the like, and their hangers or supports, shall be removed back to their source. Those which are indicated to be abandoned but are not required to be entirely removed, shall be sealed, capped or plugged at the main, branch or panel they originate from. The lines shall not be capped in finished areas, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or partitions; so that they are completely behind the finished surfaces.

To minimize interference of construction activities with flow of Medical Center traffic, comply with the following:

Keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction materials, debris and standing construction equipment and vehicles.

Method and scheduling of required cutting, altering and removal of existing roads, walks and entrances must be approved by the COR.

Coordinate the work for this contract with other construction operations as directed by COR. This includes the scheduling of traffic and the use of roadways, as specified in Article, USE OF ROADWAYS.

Construction staging area shall be level ground, and re-sod, and any damaged sprinkler heads in the staging area shall be repaired by the contractor. Staging area to be returned back to VA in existing condition prior to construction.

ALTERATIONS

Survey: Before any work is started, the Contractor shall make a thorough survey with the COR in which alterations occur and areas which are anticipated routes of access, and furnish a report, signed by both, to the Contracting Officer. This report shall list by rooms and spaces:
