About this file
This document contains a subcontractor consent form and details of a federal solicitation for repair work. The solicitation from the Department of the Air Force seeks design and construction services to repair petroleum, oil, and lubricant tanks for the White House Communications Agency at Joint Base Anacostia Bolling. Specifically, the work includes removing underground oil-water separators and storage tanks, installing new separators with integral oil storage and alarms, removing an underground diesel tank and installing above-ground diesel tanks with spill protection and connections. The magnitude of work is between $250,000 and $500,000. The acquisition is set aside for small businesses. Bids are due by a specified date, and the contract will be a firm-fixed-price award contingent on available funding. The consent form allows subcontractors to authorize the prime contractor to disclose their past performance information during the source selection process.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government's Evaluation Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The consent form must be signed by an individual who had the authority to sign for and legally bind the company. The completed consent forms should be submitted as part of your Past Performance volume. The following consent form requires editing by the prime contractor prior to forwarding to their respective subcontractors (see bolded text).
