About this file

This is a Vendor Disclosure Statement form from the Colorado Department of Corrections for contract performance outside the United States or Colorado. The form is required for prospective vendors prior to contracting or as a requirement for solicitation of any contract for services from the State of Colorado, unless the contract falls under specific exemptions including Medicare, the Colorado Medical Assistance Act, the Children Basic Health Plan Act, the Colorado Indigent Care Program, or construction contracts. The disclosure statement requires vendors to provide their solicitation number (RFP 2026), contractor name, and identify all counties and states where services under the contract and any subcontracts will be performed.

If services are anticipated to be performed outside the United States or the State of Colorado, vendors must explain why such performance is necessary or advantageous. The form requires vendor signature, date, printed name, and title upon completion and return to the contracting state agency or institution of higher education. This disclosure mechanism ensures transparency regarding the geographic location of contract performance and requires justification for any work conducted outside Colorado or the United States.
