About this file
This document is Attachment E of Austin ISD Contract & Procurement Services, which outlines the required forms that bidders must execute and upload with their proposals through the Bonfire system. The attachment specifies eight mandatory forms including the Conflict of Interest Questionnaire (with proof of online submittal), W-9 Form, Felony Conviction Notice Form, Suspension and Debarment Certification, Texas Government Code Chapter 2270/2271/2274 Verification Form, Certificate of Interested Parties Form 1295, Proposal Guarantee/Bid Bond, and Certificate of Insurance. The document provides detailed instructions for each form, including online submission requirements for certain forms like the Conflict of Interest Questionnaire and Form 1295, which must be filed electronically with respective state agencies.
The forms address various compliance and disclosure requirements, including federal suspension and debarment certifications for contracts over $25,000, Texas state law requirements regarding boycotts of Israel and energy companies for contracts over $100,000 with companies having 10 or more employees, and disclosure of interested parties and potential conflicts of interest. The bid bond requirements include specific provisions for construction projects, with original documentation required within 24 hours of bid submission. Several forms require notarization or authorized signatures from company officials, and the district maintains strict filing deadlines with state agencies, particularly for ethics-related disclosures that must be submitted within seven business days of contract execution.
