About this file
This is a cost sheet attachment for a Water Leak Detection Systems and Installation Services RFP issued by the State of Utah Division of Purchasing. The procurement solicits vendors to provide comprehensive water monitoring solutions including point-of-leak sensors, water flow sensors, multi-zoned sensor tape systems, automated remote shut-off valves, cloud-based monitoring dashboards, and installation services for state and local government entities. Two detailed scenarios are presented: Scenario 1 involves a medium-sized government office building requiring 9 point-of-leak sensors, 1 multi-zoned sensor tape system, 1 automated shut-off valve, one year of cloud-based dashboard access, pre-installation site assessment, complete hardware installation, post-installation reporting, site mapping, on-site training for 2 staff members, and a digital user manual. Scenario 2 addresses a public high school campus requiring 2 water flow sensors, 16 point-of-leak sensors, 2 multi-zoned sensor tape systems, 3 automated shut-off valves, one year of cloud-based dashboard access with AI-powered analytics, pre-installation assessment, end-to-end installation, post-installation reporting with user sign-off, site mapping across multiple buildings, training for 5 staff members, and a digital user manual. The contract term is 5 years with vendor pricing guaranteed for 1 year.
Vendors must submit all-inclusive total costs for both scenarios, with any blank yellow cells, price ranges, or zero values resulting in proposal disqualification. Each scenario is worth 150 points of the 300 available cost points. The cost scoring formula awards 100% of available points to the lowest proposed price, with other vendors receiving proportional points based on their pricing relative to the lowest bid. Part 2 of the cost sheet requires vendors to provide detailed contractual pricing breakdowns supporting scenario costs, including line-item rates, hardware unit pricing, software costs, professional services, installation, training, maintenance, and any other costs associated with the scope of work. All pricing must be fully burdened, clearly labeled with descriptions and billing terms, and include any volume or other assumptions. Vendors must attain a minimum of 70% of the 700 available technical points (490 points) to advance to cost evaluation. The procurement may result in multiple awards to vendors achieving a combined score of at least 70% of all available points (700 technical plus 300 cost). An administrative fee of 0.25% applies.

Vendor's Name:
Cost Sheet Instructions:
Part 1: Scenarios (300 points possible)
1. Required Completion: Vendors must provide a total cost for every scenario listed. If any yellow cell is left blank, contains a price range, or shows $0, the proposal will be considered non-responsive and will be disqualified from further evaluation.
2. All-Inclusive Pricing: Each total cost must include all associated expenses.
3. Cost Evaluation and Scoring: Each Scenario is worth 150 points. The cost per scenario will be used to assign cost points.
a. The lowest total proposed price will receive 100% of the available cost points.
b. All other vendors will receive a proportional percentage of the total cost points based on how their total proposed price compares to the lowest proposed price.
c. Formula:
Cost Points × (Lowest Proposed Price ÷ Vendor’s Proposed Price)
Part 2: Contractual Pricing 1.Provide all pricing that supports the total costs submitted in the scenario section. All pricing provided in this section will become contractual pricing for any awarded vendor. This list must include a complete breakdown of any line items, rates, or components that were used to calculate the total cost for each scenario.
2. Include any additional costs, fees, or rates that could be associated with completing the scope of work outlined in the RFP that may not be addressed in the scenarios. Examples include, but are not limited to: hardware unit pricing, reoccurring software costs, professional services and installation, and ongoing support, training and maintenance.
3. Clearly label each pricing line with a description that identifies what the cost represents (“Administrative Fee per Report,” etc.).
4. Indicate whether pricing is yearly, per transaction, flat fee, etc. All pricing should be fully burdened and inclusive of all costs.
5. Ensure pricing aligns with all requirements in the Scope of Work.
6. Include any assumptions or conditions related to your pricing. For example, if your pricing depends on volume, clearly state those assumptions.
7. More lines can be added if needed. You can also upload a separate pricing sheet that includes all costs in the same place you upload Attachment C Cost Sheet.
Important Notice:
If a vendor identifies any issue, ambiguity, or concern with the cost sheet, pricing scenarios, or related instructions, the vendor must submit their question or clarification request during the RFP Q&A period as outlined in the RFP Schedule. Failure to raise such concerns during the Q&A period will constitute acknowledgment and acceptance of the cost sheet and all associated pricing requirements.

| Scenarios | Details | Total Cost |
| Scenario 1: Medium-Sized Government Office Building | Background: |
A three-story government agency office building requires a proactive water monitoring solution to prevent damage to office spaces and critical IT infrastructure.
Hardware Requirements:
Point-of-leak sensors: 9 total (installed in 6 multi-stall restrooms and 3 staff breakrooms).
Multi-zoned sensor tape/ropes: 1 system (installed around the perimeter of the ground-floor IT server room).
Automated remote shut-off valve: 1 unit (installed on the facility's primary interior water main).
Software & Network Requirements:
One year of access to the centralized, cloud-based monitoring dashboard.
Configuration of automated alert notifications to designated staff.
