About this file
This is an Organizational Conflict of Interest (OCI) Statement template attachment for RFP 47QFSA26R0004, a U.S. Marine Corps MV-22 Maintenance Support contract. The attachment provides mandatory OCI declaration forms that the offeror, all subcontractors, consultants, and teaming partners must complete and sign as part of their proposal submission. The document includes two sample statement templates: one for the prime offeror and one for subcontractors, consultants, or teaming partners. Each signatory must certify either that they are unaware of any facts creating actual or potential organizational conflicts of interest, or alternatively, disclose all current information bearing on any existing or potential conflicts. The statements reference FAR Part 2.101's definition of OCI—the inability to render impartial assistance due to other activities or relationships, or situations where objectivity may be impaired or unfair competitive advantage exists. Signatories must also commit to immediately disclosing any OCI information that emerges during contract performance. The authorized point of contact for each entity signing the statement must have binding authority for their organization. This OCI statement requirement is part of the broader RFP solicitation requirements outlined in RFP Sections H.8.1 and L.5.1.4, which also reference GSA Class Deviation RFO-2025-09.

If a contractor with an actual or potential OCI believes the conflict can be avoided, neutralized, or mitigated, the contractor shall submit a copy of the new mitigation plan from the relevant awarded contract to the Government for review.
