ANNUAL ASBESTOS SURVEILLANCE REPORT
COMPLETED FOR THE
GENERAL SERVICES ADMINISTRATION – SAFETY/FIRE
PROTECTION/ENVIRONMENTAL BRANCH
A Federal Occupational Health (FOH) Contractor Representative Lisa Swain, MPH, CSP, performed a limited walk-through Annual Asbestos Visual Surveillance Check as required by the General Services Administration (GSA) for their owned facilities. Bulk or air samples were not collected. In addition, compliance with GSA contract safety and health requirements, listed on the attached checklist, was determined for Operations and Maintenance (O&M) and Custodial contractors. As indicated by GSA instructions, only spaces listed on Table A were visually checked. During the surveillance check, observations of any potential damage to the asbestos-containing materials (ACM) was noted and recorded. If any imminently dangerous conditions had been observed they would have been immediately reported to the FOH Representative, GSA Customer Service Center Manager, and GSA Safety and Environmental Management Branch Representative in accordance with GSA established procedure.
The GSA Form 3614, GSA Notice of Unsafe/Unhealthful Workplace Conditions, was completed for any potential deficiencies and provided to the GSA Building Manager.
One GSA Form 3614 may list multiple violations. An opportunity to correct violations, if any, would be given to the GSA employee or representative during the inspection.
Those violations corrected are noted in the text of the report.
The FOH Contractor Representative used the GSA ACM Checklist for fiscal year 2019, which was approved by GSA for consistency purposes and to ensure the required information was obtained at this facility. The checklist and 3614 forms, if any, are attached to this report as supporting documentation.
This report is based on personal observations during the surveillance check and from conversations with on-site personnel. Not all possible hazards and conditions may have been identified during this brief inspection, because not all spaces were inspected (per GSA), conditions may be hidden or inaccessible and changes in equipment, regulations, operations and personnel may not be known at the time of the inspection.
The report contains the following GSA required sections: Building Descriptions, General Findings, Comments and Concerns, Action Items, Form 3614, Form 3559, Corrected Items and Other.
Note: The FOH Contractor Representative designated in this project is an employee of H,S&E Resources, Inc. which has performed the services indicated in this report.
Nothing in this report is intended nor should be interpreted as establishing any form of employment or contractor relationship between the FOH Contractor Representative and General Services Administration, Federal Occupational Health, U. S. Public Health Service, or any division, agency, or contractor thereof.
Reports and other publications issued by the Environmental Health Services program of the Federal Occupational Health (FOH) were prepared under procedures established by FOH, which were designed to assure the technical competency of the persons or organizations under contract to FOH. In addition, reports and other publications were reviewed internally by one or more FOH technical staff, and all comments were addressed to the satisfaction of the issuing office, prior to dissemination.
BUILDING DESCRIPTION
The Orlando FB (FL0082ZZ) is a six-story facility plus a basement and penthouse.
The building was constructed in 1974 and renovated in 2010-2012. It has a gross area of 195,660 square feet. Five floors plus a mechanical penthouse are above ground and a mechanical basement is below ground.
The exterior surface of the building is constructed of concrete and glass windows.
The structural frame is reinforced concrete. The floors are constructed with concrete flat slabs. The interior of the building is predominantly drywall and ceiling tiles.
The prominent features of this building include six automatic elevators that service all floors, a freight elevator and government offices and courtrooms throughout the building.
The facility has fourteen Air Handling Units (AHU’s) – 2 per floor and 14 supplements units providing cooling to computer and court rooms. The AHU circulate chilled water to provide the air conditioning. Three glycol tanks, 2 cooling towers and 3 chillers supplement the cooling system. Electric strips located within each VAV units and provide heat to the building. A boiler is not present. All equipment except 2 chillers were installed during the 201-2012 renovation.
A large section of the 2nd floor was renovated in FY2016.
The O&M and custodial services are provided by Katmai.
I. GENERAL FINDINGS
An Annual ACM Visual Surveillance Check of the George C. Young Federal Building was conducted by a Contractor Representative of the U. S. Public Health Service, Lisa Swain, MPH, CSP, on October 22, 2019. Cassandra Perez, GSA Building Manager, accompanied the inspector during this survey.
During 2010-2012 the building underwent an extensive renovation. GSA personnel reported prior to the survey that all ACM had been abated during the renovation.
However, during the 2014 SEM survey the FOH representative observed 4 suspect materials remaining in the building following the renovation. These were:
Roof paper and black flashing mastic locted in enclosed penthouse
Pipe and elbow insulation in Stairwell 3
Cove base adhesive in basement room 216
Fire door insulation
In July 2018, the FOH Representative sampled the suspect materials and submitted these for laboratory analysis.
The pipe insulation was not sampled and analyzed due to the visual determination of fiberglass material. Fire doors were not sampled due to the destructive nature of sampling. Of the sample results, the black mastic on the wall in the penthouse was determined to contain 3% Chrysotile. The roof paper, cove base adhesive in the basement and black mastic on a seal in the basement were negative for asbestos.
As a result, the black wall mastic in the penthouse must be treated as containing asbestos and the fire doors must be considered as presumed to contain asbestos.
The overall building Risk Assessment Code (RAC) is 3. No significant adverse findings (RAC 1 or 2) were identified during this surveillance. Fifteen citations (15) were issued during this visual surveillance of which all 15 (15) are repeat deficiencies.
New suspect materials were not identified during this survey.
Inaccessible spaces – The following spaces were not accessible during the visual surveillance: Tenant spaces were not inspected.
Any applicable information obtained by the FOH Contractor Representative during the site visit was added to the table. The FOH Contractor Representative did not conduct any asbestos sampling or measure ACM quantities other than noting if ACM was removed since the last surveillance. The FOH Contractor Representative may note suspect materials that were not previously identified or tested and list them in the attached Table A as suspect and recommend testing.
This report has three attachments: 1) Annual Visual Surveillance Table, 2) Survey Checklist/Form 3614 and 3) Form 3559.
II. COMMENTS
Following is a list of comments and a few notable concerns in conducting the re-surveillance checks:
In July 2018, the FOH Representative sampled the suspect materials list below and submitted these for laboratory analysis.
Cove Base Adhesive in basement Black Mastic as a sealant in the basement Roof material in the penthouse Black mastic on penthouse wall
The pipe insulation, previously identified as a suspect material, was not sampled and analyzed due to the visual determination of fiberglass material.
See FOH Sampling Report dated May 2018 for additional details.
GSA’s contract has the following nine asbestos O&M requirements: 1) at least one O&M asbestos trained worker/supervisory person onsite; 2) annual fit tests; 3) annual medical exams; 4) the EPA asbestos O&M document; 5) NIBS O&M books (either available onsite or access to); 6) Asbestos repair equipment onsite; 7) written respirator program; 8) written medical surveillance plan; 9) written protective equipment plan. The O&M contractor had not met any of these requirements.
Previous survey reports, abatement records and renovation documents were available for review.
The GSA Building Manager had Asbestos awareness training.
The form notifying outside contractors performing work in the building where ACM is located was not present. Further/However, many contractors are hired by agencies and it is unlikely that these contractors are notified of the presence and location of ACM.
The annual notification to tenants and contractors regarding location and condition of ACM in the building had not been distributed by GSA.
The custodial/O&M staff had not completed the annual asbestos awareness training.
Not all the ACM could be inspected because it maybe in pipe chases, above ceilings, under carpet or behind wallpaper.
New suspect materials were not identified during this survey.
The black wall mastic was not labeled.
Signage noting the presence of ACM in the penthouse was not present.
GSA/O&M had not identified the new asbestos-free insulation installed after the recent extensive abatement. Labeling is required where ACM and non- ACM is located in the same spaces. Otherwise, a list of renovated spaces, where non-ACM insulation was installed is sufficient.
Changes in the quantity of ACM made during renovation, demolition or general O&M work had not been recorded in the O&M plan and noted in the ACM records. All changes to the quantity, condition, friability, etc. must be recorded immediately in the O&M plan. Each building should have its own records, O&M plan on site and be continuously updated by site personnel.
Each facility upon receiving this updated ACM Surveillance Table should check it for accuracy and use it to record all changes in ACM amount, location, condition, etc. This table will be requested during all future checks and audits.
IV. ACTION ITEMS
Label black wall mastic in the penthouse as ACM.
Treat PACM fire doors as asbestos containing until ACM content is identified.
Develop O&M plan to address PACM in building.
Require the O&M/Custodial contractor to meet the GSA contract requirements for asbestos until ACM content in suspect materials is determined.
A certified, asbestos trained, 16-hour O&M person with the necessary equipment must perform all minor repairs or resealing. The O&M contractor must comply with the asbestos requirements or the abatement contractor must be retained to repair/maintain/abate any ACM which is found to be damaged. Any changes to the quantity and condition of ACM in the facility should be incorporated into GSA’s Operations and Maintenance (O&M) Plan.
GSA should distribute the annual notification letter to tenants, O&M and custodial contractors regarding the status and condition of the suspect ACM.
The building materials installed during the renovation, such as a new roof, vinyl floor tile, tile and roof mastic, etc. should have documentation or a signed letter from the architect, supplier, etc. stating no ACM present in these products. The documentation should be made a permanent addition to the building’s asbestos files.
Require visiting contractors sign and date a form, usually found in the O&M plan, regarding notification of the presence of PACM before allowing any contractor to commence work in the area where the PACM is present.
Review this Annual ACM Visual Surveillance Spreadsheet (Table A) and make any necessary additions or subtractions. Bring these changes to the inspector’s attention during the next surveillance survey.
Asbestos Free stickers should be placed on all reinsulated items that once housed asbestos in order to visually display that ACM no longer is present. If all ACM has been removed from a room, then an Asbestos Free sign could be placed just inside the room to notify entrants that ACM is no longer present. When all asbestos has been removed from this facility, all signs can be taken down after a final re-inspection.
Conduct annual asbestos visual surveillance inspections at this facility. When conducting the next annual visual surveillance survey, please provide the inspector a copy of this report and request they verify location and condition.
Request the inspector to note any additional suspect ACM. Note and rectify discrepancies between the reports. All changes to condition, location, etc., should be made on the associated standardized spreadsheet.
All building materials affected by renovations and/or demolition, not previously sampled and analyzed in that specific space, must be sampled and analyzed prior to work commencing or assumed to contain asbestos.
Alternatively, obtain documentation that the building material is asbestos free.

15. Repeat - The O&M contractor did not have the EPA O&M asbestos guide, “Green Book” onsite as a guide to follow in event of an asbestos incident.
Require contractor to maintain a copy of this reference manual.
VI. CORRECTED CITATIONS
The following items were corrected during the day of the surveillance check or shortly thereafter and confirmed by the GSA Building Manager:
None
VII. OTHER
When the facility receives this updated ACM Surveillance Table, they should review it for accuracy and use it to record all changes in ACM amount, location, condition, etc. This table will be requested during all future inspections and will be audited.
When conducting the next annual visual surveillance, please give the GSA/Contractor inspector a copy of this report and have them verify all locations.
Request that the inspector note any additional suspect ACM. All inspectors must use this report and verify/correct discrepancies between it and any other reports.
Require inspectors to modify their surveillance report to include the attached spreadsheet. All changes to condition, location, etc. should be made on this standardized report.
The Surveillance Table is to be updated continuously. The table contains three color-highlighted areas that indicate changes in the amount, location or condition of ACM. Red indicates changes in the amount or condition of ACM. Blue denotes “suspect asbestos,” that should be sampled and analyzed before any renovation or demolition occurs. Green indicates that suspect asbestos was determined not to be ACM or that ACM was removed. In the “Condition” column, good implies no or very minor damage (i.e., less than 1 percent), fair implies between 1 and 10 percent overall damage and poor means damage greater than 10 percent overall or 25 percent localized.
