A Federal Occupational Health (FOH) Contractor Representative performed a limited Annual Asbestos Visual Surveillance Check as required by the General Services Administration (GSA) for their owned facilities. Bulk or air samples were not collected.

Both Operations and Maintenance (O&M) and Custodial contractors were checked for compliance with GSA contract safety and health requirements listed on the attached checklist. In accordance with GSA instructions, only spaces listed on Table A were to be visually checked. During the surveillance check, observations of any potential damage to the asbestos-containing materials (ACM) were noted and recorded. If any imminently dangerous situations had been discovered, they would have been immediately reported to the FOH Representative, GSA Customer Service Center (CSC) Manager, and GSA Safety and Environmental Management Branch Representative in accordance with GSA established procedure. The GSA Form 3614, GSA Notice of Unsafe/Unhealthful Workplace Conditions, was to be completed onsite for any potential safety and health violations. One GSA Form 3614 may list multiple violations. An opportunity to correct violations, if any, would be given to the GSA employee or representative during the surveillance. Those violations corrected are noted in the text of the report.

The FOH Contractor Representative used the GSA ACM Checklist for fiscal year 2019, which was approved by GSA for consistency purposes and to ensure the required information was obtained at this facility. The checklist and 3614 forms, if any, are attached to this report as back-up information.

This report is based on personal observations during the surveillance check and from conversations with on site personnel. Conditions, equipment, regulations, operations and personnel change, and certain conditions may be hidden, therefore, not all possible hazards and conditions may have been listed during this brief surveillance.

The report contains the following GSA required sections: Building Descriptions, General Findings, Comments and Concerns, Action Items, 3614s, Corrected Items and Other.

Note: The FOH Contractor Representative designated in this project is an employee of H,S&E Resources, Inc. which has performed the services indicated in this report.

Nothing in this report is intended nor should be interpreted as establishing any form of employment or contractor relationship between the FOH Contractor Representative and General Services Administration, Federal Occupational Health, U. S. Public Health Service, or any division, agency, or contractor thereof.

Reports and other publications issued by the Environmental Health Services program of the Federal Occupational Health (FOH) were prepared under procedures established by FOH, which were designed to assure the technical competency of the persons or organizations under contract to FOH. In addition, reports and other publications were reviewed internally by one or more FOH technical staff, and all comments were addressed to the satisfaction of the issuing office, prior to dissemination.” (This statement required by FOH.)

I. BUILDING DESCRIPTION

The R. L. Timberlake Jr. Federal Building is an 8 story concrete structure that houses several federal agencies. The building construction was completed in 1964 and the total area of the building is 113,616 square feet. The total habitable area of the building is 78,736 square feet. The building consists of eight above ground floors, with the eighth floor containing the mechanical room/penthouse. Cooling towers are located on the roof. The building is constructed with a concrete slab foundation, concrete columns and concrete beams with ribbed slabs.

The main mechanical components of the building include two chillers located in the penthouse mechanical room, one boiler in the penthouse mechanical room and two air handlers – one return and supply for each AHU on the east and west side of the eighth floor mechanical penthouse. The main pipe chases are located in the building core, adjacent to the stairwells, elevator shafts and restrooms.

The chiller and boiler rooms are located in the eighth or penthouse floor of the building. Two chillers are located on the west side of the penthouse. The boiler room is located in the middle, north side of the floor, and west of the cooling tower bay. The southeast corner of the floor contains the domestic hot water system tanks and piping. The middle, south side of the floor contains three large fan rooms and a large air mixing room.

The cold and hot water is distributed to the various floors via pipe chases and mechanical vent shafts, located adjacent to the elevators in the middle of the building; adjacent to the restrooms; and adjacent to the stairwells.

The chill water and hot water lines are directed to two large supply and two large return air handlers on the eighth floor where air is either chilled or heated in the coils

Tampa, FL and ducted to the floors below through hot air or cold air ducts. The air handlers are located in the middle, south side of the floor. The main chill water and hot water lines run from the pipe chases and out of the building, into the cooling towers, and back inside the building to the air handlers. The emergency generator room is located on the north side of the first floor.

Ceilings on all of the occupied floors are suspended grids with lay-in ceiling tiles, except for the elevator lobby, corridors, and telephone and electrical closets that were plaster. The exterior walls and core walls on all floors are plaster construction.

The interior office partition walls are of drywall/sheet rock construction. The restrooms have plaster walls and ceilings, with ceramic tile floors halfway up the walls. However, the plaster ceilings and floor tiles were removed during the restroom renovations in late 1999 through early 2000. These areas now have drop in ceiling tiles and ceramic floors.

The main corridors on the first and second floors are covered with two different types of floor tile. The remaining floor corridors are carpeted over vinyl floor tile, according to maintenance personnel. Additionally, the majority of the office spaces on the remaining floors contain vinyl floor tiles under carpeting.

II. GENERAL FINDINGS

A Contractor Representative of Federal Occupational Health (FOH), Lisa Swain, MPH, CSP, conducted an Annual Visual ACM Surveillance Check of the RL Timberlake Federal Building. The Annual Visual Surveillance check was conducted on October 23, 2019. Ms. Makena Baco accompanied the inspector during this surveillance survey. Katmai O&M personnel also provided assistance.

The Annual Visual ACM Surveillance Spreadsheet, Table A, was taken from the previous annual report and used to prepare for this surveillance check.

Any applicable information obtained by the FOH Contractor Representative during the site visit was added to the table. The FOH Contractor Representative did not conduct any asbestos sampling or measure ACM quantities other than noting if ACM was removed since the last surveillance. The FOH Contractor Representative may note suspect materials that were not previously identified or tested and list them in the attached Table A as suspect and recommend testing.

The overall building Risk Assessment Code (RAC) is 2, also noted in the FY2014 - 2018 surveys.

Three significant findings were identified during this surveillance survey. These are:

(a) Plaster is bubbling and degrading at windows in the break room, open area, and supervisor office in the 3rd floor Railroad offices. Material is friable. The windows are currently sealed with plastic. Common wall plaster has been previously identified as asbestos containing material. The space was not accessed in FY2019. And (b)

Tampa, FL

Damaged pipe insulation was observed in the penthouse inside air fan rooms. The insulation appears to be original to the building and is not labeled as asbestos free.

The room is a mixing box and it is unknown if the space serves occupied areas. And

(c) The ACM surface coating on the penthouse air-handling units continue to deteriorate.

IMMEDIATE ATTENTION IS REQUIRED FOR THESE CONDITIONS. See photographs and recommendations in Section III below.

Reportedly, a bulk sampling project was conducted in the building. However, the FOH Representative was not provided sampling data from this project.

Approximately 30 square feet of ACM flor tile was abated in the 1st floor Air Force space. Clearance air sampling for asbestos and work notes were available for review.

Disposal documents were not available.

Twenty-two (22) citations were issued during this visual surveillance of which nineteen

(19) are repeat deficiencies. Corrective measures were not taken during this surveillance survey.

Inaccessible spaces – The following spaces were not accessible during the visual surveillance: The inspector did not have access to pipe chases, above plaster ceilings and occupied tenant spaces. Only select areas above the suspended ceilings were inspected. The FOH Representative also did not have access to the 3rd floor Railroad office, 3rd floor communication room and tenant spaces on floors 2, 5, 6 and 7.

New suspect materials were not identified during this survey.

Any future changes to the quantity and condition of ACM in the facility must be incorporated into GSA’s Operations and Maintenance (O&M) Plan.

Photographs of citations and areas of concern are included in this report as available.

ACM records for the building were not available. Reportedly, previous GSA personnel maintained records and the location of records is unknown.

Three attachments are associated with this report: 1) An Annual ACM Visual Surveillance Table, 2) Checklist/Form 3614, and 3) Form 3559.

III. COMMENTS AND CONCERNS

Plaster is bubbling and degrading at windows in the break room, open area, and supervisor office in the 3rd floor Railroad office. Material is friable. Common wall plaster has been previously identified as asbestos containing material. Debris on window sill observed. IMMEDIATE ATTENTATION IS REQUIRED. The windows are currently sealed with plastic. Reportedly, the O&M personnel improved seal on the windows following the site visit. Photographs are from previous surveys. Removal or disturbing of the plastic covering could result in a potential asbestos release. The space was not accessed in FY2019. This condition in considered a RAC 2.

Note: HEPA vacuuming of debris would normally be recommended, however the material is so fragile, the FOH Representative is concerned about any type of disturbance in the area of damage.

Damage was observed on piping that appeared original to construction in the outside air fan units. Sampling data for this material was not available and the insulation was not labeled non-ACM. The ACM content is not known, therefore sampling and analysis is required. Until results are received the material must be treated as ACM. The FOH representative was unable to determine if this airflow is mixed and supplied to the occupied spaces or simply serves the penthouse. If these units housing the damaged insulation is ACM and does mix with the air supplied to the occupied spaces, GSA should conduct air monitoring of affected tenant spaces. As previously noted, ACM bulk sampling was conducted in the building during 2018, however sample locations were not identified on this material during the 2019 survey. Sample results were not available for review.

Further, the O&M personnel who are not properly trained or equipped, conducted repairs using duct tape.

GSA must demonstrate that this material is non-ACM and if so, is not being distributed through the air flow into occupied spaces. This condition in considered a RAC 2.

Widespread damaged ACM mud on AHU's was observed in the penthouse. This condition was noted in the previous ACM reports. One area had been sealed with fiberglass board in 2016 and as a result the condition was downgraded to RAC 3 from RAC 2 in FY2016. However, since that time the surface coating continues to deteriorate and has once again been elevated to a RAC 2.

Tampa, FL

Damage was observed on the domestic hot water tank, boiler breaching and piping in the penthouse.

Damaged wall plaster in 1st floor janitorial closet and 1st generator room, east side of 3rd and 4th floor (vacant space). Photograph from previous survey.

Tampa, FL

Black mastic on silver foil duct insulation under the west stairwell is in poor condition.

The ceiling plaster in the 3rd floor janitors’ closet is in poor condition. The door was sealed during the survey and must remain sealed until abatement is implemented. Debris observed on floor. Photograph from previous survey.

Opening the space could result in release of asbestos fibers.

Tampa, FL

Floor tile in the maintenance shop hallway and 2nd floor storage room is dull and scuffed.

PACM acoustical wall panels in the 4th floor computer room are frayed and damaged.

PACM floor tile in the fire pump room is dull and scuffed.

Asbestos labels were missing in multiple locations.

Asbestos documentation is missing and incomplete.

The O&M contract has the following nine asbestos O&M requirements: 1) at least one O&M asbestos trained worker/supervisory person onsite; 2) annual fit tests; 3) annual medical exams; 4) the EPA asbestos O&M document; 5) The NIBS O&M book (either available onsite or access to); 6) Asbestos repair

Tampa, FL equipment onsite; 7) written respirator program; 8) written medical surveillance plan; 9) written protective equipment plan. The O&M contractor had provided the 16-hour ACM training to the supervisor and had the EPA Green book on site.

However, ACM refresher training had not been provided to other O&M personnel.

New suspect materials were not identified during this survey.

The annul notification letter regarding current location, status and condition of asbestos within the building had not been provided by GSA to the tenants and contractors.

Custodial personnel had not been provided annual asbestos awareness training.

They had low speed buffers and HEPA vacuums on site for use.

A contractor notification and sign-in sheet regarding the presence of ACM within the building was not present and implemented by the O&M contractor.

Carpet has been installed over VAT in several spaces. If carpet is adhering (glued) to floor tiles, carpet removal must be considered an abatement project.

Not all the ACM could be inspected because it maybe in pipe chases, above ceilings, under carpet, behind wallpaper or in occupied tenant spaces.

IV. ACTION ITEMS

Immediately sample friable damaged plaster in Railroad office. If ACM is identified, conduct air sampling to determine potential occupant exposure.

Immediately abate. Re-location of tenants may be required. Prohibit any disturbance of the material until sampled and analyzed. Ensure plastic coverings over windows are maintained. Do not disturb the material until sampled and analyzed

Immediately sample pipe insulation in penthouse fan room. If ACM is identified, determine if occupied spaces are served by the fan room. If occupied spaces are affected, conduct air sampling to determine potential occupant exposure. Fan room is mixing box. Abate or repair insulation.

Abate/seal/repair damaged ACM mud insulation on AHU immediately.

Repair/seal damaged plaster. Use caution when making penetrations in plaster.

Ensure appropriate training and PPE for personnel disturbing plaster.

Abate/repair other damaged ACM noted above in accordance with State and Federal guidelines.

Tampa, FL

Continue to restrict access to 3rd floor janitor closet. Abate damaged ceiling plaster.

Seal PACM acoustical wall panels.

Maintain seal wax on ACM and PACM floor tile or abate.

Maintain asbestos containing building materials in good condition.

Ensure GSA Building Managers and Specialists participate in the GSA online asbestos awareness training. Maintain a copy of the training records in building asbestos file.

A certified, asbestos trained, 16-hour O&M person with the necessary equipment must perform all minor repairs or resealing. The O&M contractor must comply with the asbestos requirements or the abatement contractor must be retained to repair/maintain/abate any ACM which is found to be damaged. Any changes to the quantity and condition of ACM in the facility should be incorporated into GSA’s Operations and Maintenance (O&M) Plan.

Require O&M and Custodial contractors to meet all the nine (9) requirements of the asbestos portion of the O&M contract. The O&M contractor must not perform repairs or abatement unless they are properly trained and equipped. Installation of duct tape on damaged PACM insulation is not considered appropriate repair activity.

Changes in the quantity of ACM made during renovation, demolition or general O&M work had not been recorded in the O&M plan and noted in the ACM records. All changes to the quantity, condition, friability, etc. must be recorded immediately in the O&M plan. Each building should have its own records, O&M plan on site and be continuously updated by site personnel. Each facility upon receiving this updated ACM Surveillance Table should check it for accuracy and use it to record all changes in ACM amount, location, condition, etc. This table will be requested during all future checks and audits. Abatement documents must be maintained on site.

Ensure GSA Building Managers and Specialists participate in the GSA online asbestos awareness training. Maintain a copy of the training records in building asbestos file.

Maintain all asbestos repairs, abatement, sampling and inspection documents on site for review.

Maintain labels and signs on all accessible ACM.

Tampa, FL

Distribute the annual notification letter to tenants and O&M/Custodial contractors regarding status, condition, presence and location of ACM in the building.

The facility should review the attached updated spreadsheet (Table A) ACM Surveillance Table for accuracy and use it to record all changes in ACM amount, location, condition, etc. This summary table will be requested during all future inspections and be audited.

Utilize HEPA (high efficiency vacuum cleaners where dust and debris possibly originating from areas of asbestos are an issue.

Require outside contractors sign and date a form regarding notification of the presence and location of ACM before allowing any contractor to commence work within the building. This document demonstrates communication between the contractor and GSA regarding ACM and procedures for working around ACM.

GSA should ensure that agencies hiring contractors also provide notification.

All building materials affected by renovations and/or demolition, not previously sampled and analyzed in that specific space, must be sampled and analyzed prior to work commencing or assumed to contain asbestos. Alternatively, obtain documentation from the manufacturer stating that their product contained no asbestos. Maintain records in the asbestos file to be used for future inspections and surveillances.

Carpet has been installed over VAT in several spaces. If carpet is adhering (glued) to floor tiles, carpet removal must be considered an abatement project.

Previous analyses of floor tiles nd mastic with negative results are suspect if analyzed by PLM. TEM is the current method of choice because it can see the coated asbestos fibers that PLM cannot. Also, new floor tile and mastic may contain asbestos depending on overseas manufacture location. Therefore, floor tiles that were negative for asbestos and newer floor tile should be re-sampled and analyzed by TEM before renovating any area containing floor tile.

Any new or future renovation or remodeling must have a letter from the architect and/or contractor stating no ACM building products were reinstalled. Otherwise all building materials must be considered Suspect ACM and be tested for asbestos before being disturbed. GSA must perform a pre-renovation check as per the GSA asbestos policy.

Suspect ACM must be treated as ACM until sampling and analysis is completed.

Asbestos Free stickers should be placed on all reinsulated items that once housed asbestos in order to visually show that ACM is no longer present.
