About this file

This is a Contract Modification #2 and Extension #1 document between the Board of Franklin County Commissioners and Metropolitan Life Insurance Company for the provision of voluntary group short and long-term disability insurance and related benefits to county employees under the Franklin County Cooperative Health Improvement Program. The original contract was executed on January 18, 2022, under RFP #2020-06-35, with a First Amendment added on December 18, 2023, to include group legal services plan benefits. This extension establishes a contract term from January 1, 2025, through December 31, 2026, representing a 24-month extension period with no additional extension periods remaining. The modification adds new benefit offerings including employer-paid group legal services plan benefits at $8.25 per month, pet insurance as an entirely employee-paid benefit, and potential identity theft insurance coverage if the county elects to offer it.

The not-to-exceed amount for this extension is $185,000.00. Pricing for voluntary long-term disability coverage ranges from $0.351 to $0.627 per $100 of covered monthly payroll across age bands, representing a 1.1 percent decrease from current rates and guaranteed through December 31, 2026. Voluntary short-term disability rates range from $0.282 to $0.629 per $10 of covered weekly benefit, reflecting a 4.2 percent reduction. Employer-paid short-term disability alternative plan options are available with rates ranging from $0.136 to $0.238 per $10 of covered weekly benefit depending on the plan selected. The employee-paid group legal services plan participation fee remains $16.50 per month, while identity theft insurance rates, if adopted, range from $3.00 to $12.95 per employee per month depending on plan selection. Metropolitan Life Insurance Company's notice contact has been updated to Charlie Cross, Senior Account Executive, located in Southfield, Michigan.
