About this file

This is a benefit program comparison variances document for Franklin County Cooperative's RFP 2025-06-09 related to health insurance and disability program administration. The document outlines proposed changes to current benefit structures across multiple employee groups including hourly HMO, new hire hourly HMO, salaried HMO, salaried PPO HDHP, salaried student PPO, and student HMO plans. The RFP addresses comprehensive health and welfare benefits administration, including medical, mental health and substance abuse coverage, pharmacy services, disability programs (STD/LTD), legal services, pet insurance, and newly added ID theft insurance. The document serves as a template for bidders to identify and document all plan deviations from current offerings, with the understanding that additional variances may emerge during implementation review processes.

Key variances identified in the HMO plans include restrictions on preferred specialty drug supply limits (reduced from 34-90 days to 30 days maximum), inability to accommodate current mental health and substance abuse benefit structures due to payment policy constraints and potential Mental Health Parity Compliance issues, and modifications to legal services benefits (reduced from 4 sessions to 3 sessions). The incumbent carrier's current plan designs must be maintained unless specifically identified as deviations in this document. Bidders are required to provide all programs, services, and administration currently offered unless explicitly listed as plan deviations, ensuring continuity of coverage while allowing for identified modifications and new benefit additions.
