About this file

This is a Contract Modification #2 and Extension #1 document between the Franklin County Board of Commissioners and Metropolitan Life Insurance Company for the continuation of voluntary group short and long-term disability insurance, group legal services, and ancillary benefits for County employees. The extension period runs from January 1, 2025 through December 31, 2026, representing a 24-month extension with no additional renewal periods remaining. The contract was originally executed on January 18, 2022 under RFP #2020-06-35, with a first amendment added on December 18, 2023 to include group legal services. The modification adds pet insurance as an entirely employee-paid benefit effective December 1, 2024, establishes provisions for optional identity theft insurance coverage, and updates the contractor's notice contact information to Charlie Cross, Senior Account Executive, located in Southfield, Michigan.

The total not-to-exceed amount for this extension is $185,000.00. Long-term disability renewal rates for the 2025-2026 period are guaranteed with a 1.1 percent decrease across all age bands, ranging from $0.351 to $0.639 per $100 of covered monthly payroll. Short-term disability rates are guaranteed with a 4.2 percent reduction, ranging from $0.282 to $0.629 per $10 of covered weekly benefit. The employee-paid group legal services plan participation fee remains $16.50 per month, while a new employer-paid participation fee of $8.25 per month is added. Identity theft insurance options are available at guaranteed rates for three years, ranging from $3.00 to $6.00 per employee per month for employer-paid plans and $5.55 to $7.95 for voluntary employee-paid plans. Pet insurance rates are determined by the specific plan selected by employees. All policy discounts continue to apply to the contractor's rates for benefits-eligible employees.
