About this file

This is a Cost Proposal Attachment Form for Salt Lake City Corporation's Request for Proposals (SLCI27013) for an AI-enabled non-emergency public safety call handling and assistive solution for the City's Emergency 911 Department. The cost sheet template is designed for offerors to submit comprehensive pricing for the purchase and implementation of the proposed system over an initial five-year contract term. Offerors must provide costs across multiple categories including initial setup and licensing, annual maintenance, product costs for years one through five, implementation fees, user training, configuration and customization services, and any additional optional future enhancements or value-added services. The form requires detailed breakdowns of hourly rates for configuration, training, developer customization, and other professional services that may be required during the contract lifecycle. All cost entries must align with the technical requirements and specifications outlined in the RFP, and any costs not documented on this form will not be permitted in the resulting contract.

The evaluation of proposals will be based on the lowest total proposed costs, which combines initial implementation and maintenance costs with ongoing annual expenses throughout the five-year term. Offerors must complete all yellow-highlighted cells with proposed pricing or enter "N/A" if a cost category does not apply; blank cells or zero entries will be interpreted as costs being included in another line item or provided at no cost. The form explicitly requires that all costs necessary to implement and maintain the system over the initial contract period be included, and any deviation from the specified format may result in proposal disqualification. Offerors are permitted to add additional lines for modular pricing, configuration services, training costs, and value-added services not captured in standard categories, with detailed product descriptions and cost drivers required for all supplementary offerings.

Offeror Name:

Costing Considerations & Instructions
1. All cells in a yellow highlight allow for a Offeror input.
2. Cost will be evaluated based on the lowest Total Proposed Costs (cell H43 and Highlighted in GREEN) for the purchase and enhancements required to meet the specifications of this RFP. Total Proposed Cost is a combination of cell H29 & D40 (both highlighted in Blue).
3. Technical Requirements and Cost Proposals must be reflective of one another. All costs necessary to implement and maintain the proposed system over the initial contract's five (5) year life cycle must be included on this Cost Schedule form.
4. If zero is entered for any cell or if any cell is left blank, the City will assume that the related services are either provided at no cost or built into another cost item. If a cost category is not applicable, enter N/A.
5. Where Offeror's proposed solution incorporates pricing or licenses for different modules, the pricing for the separate modules shall be listed on separate lines below. As needed, Offeror may add additional "Other Costs" lines.
6. Any cost not provided on this Cost Schedule, will not be allowed into the resulting contract. If you identify a cost that cannot be properly captured in this Cost Schedule, then provide suggestions for modifying this Cost Schedule to the Q&A board for this solicitation.
7. Any deviation from this format may result in disqualification of Offeror's proposal.
