About this file

This is a Cost Proposal Form for Salt Lake City Corporation's Request for Proposals SLCI26052 for Volunteer Management Software. The solicitation seeks a software solution to manage volunteer programs throughout the city. Offerors must submit cost proposals using the provided attachment, which establishes the pricing structure for the initial five-year contract term. All costs necessary to implement and maintain the proposed system over the five-year lifecycle must be included on this cost schedule form, with pricing broken down by year (Year 1 through Year 5). The cost sheet requires offerors to provide itemized costs for licensing based on 15 licenses for the city, annual maintenance fees if not included in licensing, initial user training, and implementation setup costs. The contract term is five years, with no renewal options mentioned in this document.

Pricing evaluation will be based on the lowest total proposed costs, calculated as the combination of implementation and annual maintenance costs plus any configuration, training, and optional future enhancement costs. All cells highlighted in yellow allow offeror input, while total proposed costs are highlighted in green for evaluation purposes. Offerors may propose additional modules and separate lines for other costs, and must include hourly rates for configuration, training, developer customization, and additional training services if applicable. If any cost category is not applicable, offerors must enter "N/A"; if zero is entered or cells are left blank, the city will assume those services are provided at no cost or built into another line item. Any cost not provided on the cost schedule will not be allowed into the resulting contract, and any deviation from the required format may result in proposal disqualification. The form also requires offerors to describe value-added services or alternative solutions with corresponding product descriptions, cost drivers, and pricing.
