About this file

This is a Debarment and Suspension Certification document required by the Cherokee County School District (CCSD) Procurement Services Department for an HVAC Air Filter Services RFP in Georgia. The certification mandates that all bidders and their proposed subcontractors must not be debarred, suspended, ineligible, or excluded from contract awards by the U.S. Department of Agriculture or any other Federal, State, or Local Agency. CCSD is soliciting proposals from qualified air filter service providers to supply customized HVAC air filter installation and replacement services district-wide on a quarterly basis. All proposed air filters must meet a minimum rating of Merv 8 or Merv 9. Award will be made on an "all or none" basis, meaning the selected proposer will manage filter change-outs across all CCSD locations. All inquiries regarding this RFP must be submitted through the Euna Procurement Message Center.

Bidders must execute the debarment and suspension certification under penalty of perjury, and any bidder unable to provide this certification will be deemed ineligible for award consideration. Bidders are required to immediately notify CCSD if they become debarred or placed on the Consolidated List of Debarred, Suspended, and Ineligible Contractors by a Federal entity at any point during the bid process or contract period. All bidder responses will be verified against debarment and suspension databases. The certification was last updated in August 2023.
