About this file

This document contains a past performance questionnaire for a federal contract solicitation. Bidders are required to provide past performance references for at least five recent National Cemetery Administration customers over the past three years involving grounds maintenance services such as upright headstone raising and realignment, headstone cleaning, and topsoil/turf renovation. References must include the contract number, project title and description, total contract value, period of performance, customer name, and point of contact information. The National Cemetery Administration will review the references based on criteria of quality performance, contract timeliness, cost control, and business relations, and may also send out past performance questionnaires to the listed contacts and review CPARS reports. The related federal contract opportunity is for grounds maintenance services at Midwest District National Cemeteries under Solicitation Number 36C78623B50188 issued by the Department of Veterans Affairs National Cemetery Administration.
