About this file

This is an Asbestos Assessment Report Addendum prepared for the Department of Veterans Affairs, VA Montana Healthcare System, for the Veterans Administration Medical Center in Miles City, Montana, covering Buildings 1, 8, and 20.

The addendum, dated April 20, 2013, modifies the original asbestos assessment completed in March 2011 by providing additional inspection and testing. The original assessment assumed the presence of asbestos-containing floor tile in the Fourth Floor "Victory Insurance" area (Room Numbers 407A, 418B, 425, 425A, 426, 431, 432, 433, 433A, 433B, 434, 434A, 434B, 435, 437, and 438). On March 21, 2013, certified asbestos inspector Tom Heinecke conducted a follow-up inspection and found no floor tiles present in these areas; however, mastic was identified and sampled. Laboratory analysis confirmed no asbestos was detected in the mastic samples. Additionally, the original assessment had identified Tan HVAC Mastic (Material H1.1) in Room #451 without sampling. Heinecke re-inspected this area, reclassified the material, and collected three samples for testing. Results confirmed no asbestos was detected. The analytical testing was performed by Cardno ATC laboratory using EPA Method 600/R-93/11 (PLM analysis), with all six samples analyzed returning results of "None Detected" for asbestos content. The inspector credentials confirm Tom Heinecke's Montana Certification Number MTA0079 and Accreditation Number I-88-1-19.

Morrison-Maierle Project #5218.001

Executive Summary:

This Addendum is intended to modify the original assessment of the building by providing additional inspection and testing. All other assessments remain unchanged.

The original asbestos assessment prepared by CTA Construction & Environmental was prepared in March of 2011 and assumed asbestos containing floor tile to be present in the Fourth Floor Area defined as “Victory Insurance”. The rooms are identified as Room Numbers 407A, 418B, 425, 425A, 426, 431, 432, 433, 433A, 433B, 434, 434A, 434B, 435, 437, and 438.

On March 21, 2013, Tom Heinecke, a certified asbestos inspector, inspected the floor in the areas listed above and did not find any floor tiles. Mastic was present, however.

The mastic was sampled and sent to a certified laboratory for analysis. The results of the testing indicate no asbestos was detected.

The original asbestos assessment prepared by CTA Construction & Environmental identified Tan HVAC Mastic (Material Identification Number H1.1) to be present in Room #451. In the CTA Assessment, no samples were taken of this material from this Room.

It is believed that there was a simple mistake in identifying the material.

On March 21, 2013, Tom Heinecke, a certified asbestos inspector inspected this area and determined that the material identified as Material H1.1 should have been classified as a different material. Three samples were taken of the material and sent to a certified laboratory for testing. The results confirm that no asbestos was detected in the material.

As a result of the actions outlined above, no asbestos containing flooring materials are present in the “Victory Insurance” area of the building AND no asbestos containing materials are present in Room #451 of the building.

The name, signature, and certification number of the inspector who prepared this report is listed below:

These samples were analyzed by layers. Specific layer or component asbestos content is indicated when relevant. The EPA considers a material to be asbestos containing only if it contains more than one percent asbestos by Calibrated Visual Area Estimation (CVAE). EPA regulations also indicate that Regulated Asbestos Containing Materials (RACM) -- materials which are friable or may become friable -- be further analyzed by point counting when the results indicate less than ten percent asbestos by CVAE. Our laboratory utilizes CVAE on a routine basis and does not include point counting unless specifically requested.

The results may not be reproduced except in full, and should not be used as a scope of work for abatement without consulting with Cardno ATC.

PLM samples were analyzed utilizing the Environmental Protection Agency's Test Method: Method for the Determination of Asbestos in Building Materials (EPA 600/R-93/116. July, 1993). Reporting Limit <1% Asbestos. Additional treatment and tests may be required to accurately define composition (i.e. ashing, extractions, acetone treatment, andTEM). Unused portions of samples are archived for one year unless client requests special handling.

Asbestos content of mastic/adhesive is separated from total percent asbestos and other materials.

Laboratory analysis was accomplished utilizing an Olympus BH-2 polarized light microscope. The microscope is equipped with dispersion staining lenses.

Cardno ATC is accredited by NVLAP Bulk Asbestos Sample Quality Assurance Program (Lab Code 102031). Cardno ATC participates in the NVLAP Bulk Asbestos Sample Quality Assurance Program and maintains an in-house QC/QA program for bulk samples whereby 10% of all submitted samples are reanalyzed and documented in a Quality Control Manual. Cardno ATC also participates in a quarterly round robin QC/QA program for bulk samples with several accredited laboratories throughout the United States. Current and past QC/QA program results are available in the laboratory for inspection.

Samples were analyzed by Jeff Lomme, Laboratory Director. Mr. Lomme is a professional geologist who has successfully completed the McCrone Institutes's "Advanced Asbestos Identification" Course.

Approved Signatory :

The non-detection of asbestos fibers in floor tile by PLM is of itself inconclusive. Confirmation by Transmission Electron Microscope (TEM) is recommended for negative floor tile samples.

This report must not be used by the client to claim product endorsements by NVLAP or an agency of the U.S. government.

This test reports only to the items stated.
