About this file
This document is a Field Observation Report and associated inspection records for the Audie Murphy Memorial Veteran's Hospital 2nd Floor Roof Replacement Project (Owner Project No. 671-21-143, WestEast Design Project No. 23040).
The field visit conducted on June 13, 2023 by ELK Consortium-West East Design Group JV, LLC identified critical roofing defects during investigation of misaligned solar panel posts. Key observations include: (1) a solar panel post penetration was selected for core investigation after appearing most out of alignment; (2) roofing system thickness was determined through separate core sampling; (3) roofing cutout materials were reinstalled with liquid flashing applied; and (4) a significant defect was discovered where the pipe design allows water to enter the pipe sleeve down to an open base plate, with a temporary corrective measure recommended involving a sheet-metal collar and sealant application at the top. The Tremco Roofing & Building Maintenance core inspection report documents the roof system composition as vapor barrier with 1" PolyISO, 1/4" wood fiber cover board, and built-up roof system with aggregate surfacing, measuring approximately 8" from surface to metal deck. The Lundy & Franke Engineering field report notes that the solar panel post installation penetrated through lightweight concrete to attach to the structure, with replacement lightweight concrete or high-strength nonshrink grout used as patching. Site personnel included VA's Chief of Design, VA Contracting Officer Representative, and representatives from the roofing contractor, structural engineer, and design firms. The investigation provided means and methods documentation for post installation analysis, with follow-up roofing work to include Alphaguard BIO liquid applied flashings on all roof penetrations for the future roofing project.

WestEast Design: 23040 REPORT SUBMITTED DATE 06/20/2023
Work
1. Uncovering of the roofing at solar panel post.
VA+ A&E on Site
1. Roger DeVeaux, Chief of Design
2. Rodrick Thomas, VA COR
3. VA’s M&O
4. Joe Fiala, Tremco Rep
5. Jose Hernandez, Lundy & Franke
6. Jon Quebe, WestEast Design
Observations
1. A solar panel post was selected that appeared the most out of aligned was determine for the core investigation. (Fig. 1,2,3,4)
2. A separate core was done to determine the thickness of the roofing system. (Fig. 5,6)
3. Roofing cutout materials was put back then liquid flashing was applied. (Fig. 7,8)
4. Discussed with Roger DeVeaux where water was getting into the roof.
a. The pipe design is backwards which allows water to enter the pipe sleeve down to the base plate which is opened. (Fig. 9, 10) Recommend applying sheet-metal collar and sealant at top for a temporary fix.

FIELD REPORT
DATE: 6/13/23
TIME: 10 am
WEATHER: partly cloudy/overcast
ARCHITECT: West East Design
PERSONNEL PRESENT: Jose H(LFE), Jon Quebe(W/E), Joe w/ Tremco
OBSERVATION: Solar Panel Post installation verification
Notes: Picture Order by Note #
1. Roofing material and rock was removed to expose lead layer of roofing material.
2. roofing lead layer peeled back to expose mastic layer and other roofing materials.
3. see note 2
4. roofing materials removed down to lightweight concrete layer
5. example of base plate found on site.
Add’l Observation Notes:
Based on the approximate 12” square removal of roofing materials of one stand alone post, we observed that for this post, the installation penetrated though the light weight concrete, leading us to believe they removed a portion of the concrete over the deck and structure and attached to the structure and patched over the deck with replacement lw concrete or high strength light weight non shrink grout. This one selective demo does not verify all posts were done the exact same way but gives us an idea of means and methods used.
