About this file

This document contains a past performance evaluation questionnaire and information on a related federal contract opportunity for design-build services. The past performance questionnaire is intended to collect feedback from references on a contractor's performance on previous similar projects. It asks evaluators to rate the contractor's compliance, management, personnel use, timeliness, schedule adherence, safety responsiveness, customer relations, problem-solving abilities, financial management, responsiveness to notices, and overall satisfaction on a previous job on a scale of exceptional to unsatisfactory. The related federal contract opportunity is a solicitation for design and construction services for a parking garage at the Temple VA Medical Center in Temple, Texas. Services required include design, construction, site work, demolition, and project completion within 365 calendar days of the notice to proceed. The estimated cost range for the construction is between $5-10 million. The opportunity is set aside for Service-Disabled Veteran-Owned Small Businesses. Responses are due June 29, 2018 with an anticipated award date in April 2018. The sponsor is the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 17.
