About this file
This document is an Electrical Equipment Test Report prepared by Electric Power Systems for S & B Services, documenting preventive maintenance testing performed at the VA Hospital in Poplar Bluff, Missouri. The report covers testing of nine automatic transfer switches, with a comprehensive infrared (IR) scan of electrical equipment to assess functionality, safety, and reliability. No significant electrical defects were identified during the inspection, though one recommendation was made to eliminate a contactor and wire directly to the breaker for a hot water pump system.
The related federal contract opportunity is Solicitation Number 36C25525R0136 for Project 657A4-25-107, which involves Triennial Electrical Testing at the John J. Pershing VA Medical Center. The solicitation is issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 15, located at 1500 North Westwood Blvd., Poplar Bluff, Missouri 63901. The project encompasses electrical system testing, with multiple amendments providing updates to the Statement of Work, bidder questions, and pre-bid conference information.

Purpose:
Electrical systems begin to degrade at the onset of installation and operation. There are many factors, such as design intent, environmental conditions, shipping / handling and installation practices to consider regarding electrical equipment related to power system safety and reliability. Maintenance testing of electrical equipment and apparatus is a useful tool in determining the continued condition of electrical equipment and systems.
Maintenance testing procedures aid in providing critical information as to the functionality, safety and reliability of electrical equipment and systems as a whole, as well as providing baselines for continued monitoring of the lifetime of the electrical equipment. This will aid in planning for future replacement or upgrading.
The equipment and components listed in this report have been inspected and tested to help assure their proper and safe operation. Defective or marginal equipment can be identified, repaired, replaced or scheduled for future repairs without costly or unexpected interruptions during normal operating periods

Summary:
The contact resistance creates impedance in the path of current flow. This impediment causes a voltage drop in the circuit. Thus, the receiving end voltage reduces with an increase in contact resistance. Resistance causes some of the electrical energy to turn into heat, so some electrical energy is lost along the way.
The importance of the equipment and the process involved in repair should also be considered when scheduling maintenance shutdowns. The following color codes show the severity of each issue and how soon it needs to be attended.
Red – High priority, Repair immediately Orange – Medium priority, Repair at your convenience or next scheduled shutdown Green – Low Priority, Repair as time permits Blue - Informational
Chiller #1 – The switch contact resistance is high on C phase.
New Switchgear Building – ATS-4 & ATS-MRI breakers have a factory recall on them and will be replaced later by the manufacturer. No testing was performed.
Building 3 - 3A Disconnect – B phase contact and fuse holder resistance is out of tolerance.
3B Disconnect – Multiple contact resistance points are out of tolerance.
Building 4 - 4A Disconnect – Multiple contact resistance points are out of tolerance.
4B Disconnect – Multiple contact resistance points are out of tolerance.
Building 7 - 7P1 Disconnect – When the switch is open, the door is not latched closed. There is a broken door latch that needs to be replaced.
Panel P-1 – This panel does not have a neutral pulled to it and is using the ground bond for all of its neutral connections. This is causing issues downstream and needs to be repaired.
12 | P a g e
Chiller #2 - C phase contact resistance is out of tolerance.
MCC-3 - MCC-3 Disconnect – Contact resistance on A and B phase main contacts are out of tolerance.
Bucket A/C 1 – There is a 14-gauge wire that is fed from a 40 amp breaker. Max 14-gauge protection should be 15 amps.
EMCC-1 – Some equipment downstream from EMCC-1 would not start up and was showing odd voltages. We tested the voltage at EMCC-1 and read AB-207, BC-35 & CA-185. We then tested another panel fed from the same switchboard that EMCC-1 is being fed by and it read correctly (209 vac on all). Checked the breaker feeding EMCC-1 and it was reading wrong. We operated the breaker about 4 times, and it started operating correctly. Recommend replacing the EMCC-1 feeder breaker.
Transformer T3 – There is no pressure gauge or nitrogen top off valve to see the pressure or fill the nitrogen. Recommend adding both. EPS sells and installs a kit for this.
13 | P a g e
Padmount Switch T1, T3, T5 & T7 – This padmount switch has a main switch and 5 feeder switches. The individual switches cannot be shut down without the main switch being shut down.
This is more of a junction box to split to all the transformers. There is nothing wrong with this, this is just a note. Transformer T10 is also fed from this switch box. T10 is still energized but is no longer being used.
ATS-4 – The exhaust fan for the emergency generator is fed from ATS-4. This needs to stay on when the generator is running. Recommend moving this circuit to a UPS to avoid any future issues. ATS-4 was not tested.
Generator 1 Transfer Issue – If Generator 1 is on a load test with any of the ATS switches and a true power outage occurs, the Generator will not shut down automatically, even after all loads have been transferred back to normal power. The generator will have to be manually shut down and reset.
New MV Switchgear After closing the breaker for T1, T3, T5 & T7 after completing the work for the night, we realized the relays were not showing voltage. The PT fuse was plugged in, but it was upside down in the Off position. After flipping it over to the On position, the voltage showed up on all the relays.
MV Breaker T11-T12 – The breaker would not close remotely with safety remote control or with the pistol switch. We found two wires crossed in the gear on a MOC switch.
After swapping the wires, the breaker functioned as expected.
1B Main Preferred Breaker – After racking this breaker out, we noticed that the C phase finger cluster had a bent finger. The cluster was removed from the breaker and the finger was straightened out using a vice and reinstalled on the cluster. The cluster was put back on the breaker for continued service. The breaker was racked in and out to verify that the damage hadn’t occurred in the cell. This most likely happened during shipping or moving.
