REQUEST FOR INFORMATION (RFI) RESPONSES
The following questions and answers provide clarification to the contract documents:
36C252-18-R-0104 Construction MATOC – Clement J. Zablocki VAMC, Milwaukee, WI Contractor Requests for Information
42. Follow on question and answer for RFI #6 of Amendment 00002: The first statement is asking for a site specific safety plan per Spec. 01 35 26. If we are to give a Site Specific SP per 01 35 26 will far exceed 25 pages. On past projects at the Milwaukee VA our site specific Safety Plan (Accident Prevention Plan) is anywhere from 220 pages to 250 pages dependent on the project. Showing a site specific Safety Plan per Spec 01 35 26 would be impossible in 25 pages even if summarized. To Clarify… If we include the pages required, this means that when we exceed the 25 pages with our Safety Plan does that mean the additional pages in V1 Part1 (Tab 3) are not counted in just Tab 3 (Meaning approx. 225 pages won’t be considered if we are to submit an Accident Prevention Plan per 01 35 26) or will Tab sections 5,6 & 7 will not be considered that follow that? Also…If a site specific per Spec 01 35 26 is required is it Site Specific to the SEED Project or the Milwaukee Campus in general? Can the page count for Tab 3 be increased to 250 pages so we can include a Safety Plan per Spec section 01 35 26 and adequately be evaluated?
Response: The response to RFI #6 of Amendment 00002 is hereby rescinded and replaced with the following: Offerors shall submit a safety plan which addresses the items listed in the basis for award and evaluation factors section of the solicitation. Upon award of a task order, the awardee will be required to submit a site specific safety plan in accordance with VA Specification 01 35 26. Page limitations for Volume 1 Part 1 are hereby reverted back to 25 pages. Tabs 4, 5, 6 & 7 are to be included in the page limitation and will not be removed if the page limitation is exceeded. If the page limitation is exceeded, pages in excess of the limit from the following tabs, in the order provided, will not be read or considered: Tab 3, Tab 2, Tab 1. All references of 50-page limit in the response to RFI #38 of Amendment 00002 are hereby changed to read 25-page limit.
43. For the 2 split systems they are calling for the 2 indoor units to be 110V on the schedule. Mitsubishi does not make this model in 110V it would be 208v 1PH. An electrical sub panel would need to be added for this. Please clarify.
Response: The two indoor cooling units should be 208 Volts. On Drawing MM500, on the Room Cooling Units Schedule, in the column marked Volts, Replace 120 with 208 for both indoor units listed.
44. In response to the answer to question 37 on amendment 00002 “ added spec section 03 07 60 FRP concrete”. How does this relate to the concrete pad? Is this not typical cast in place concrete? Are there other areas we should be aware of that we do not see that would require FRP Concrete?
Response: Milwaukee VAMC’s standard work process is to use Fiber reinforced concrete for slab on grade, non-structural concrete. For the seed project the only poured concrete are these 2 concrete pads at 5 & 6.
